云南省玉溪地区汉族居民糖化血红蛋白水平分布情况调查 被引量:2

Study on the distribution of glycosylated hemoglobin in the population of Han nationality in Yuxi area,Yunnan Province

摘要 目的探讨云南省玉溪地区汉族居民HbA_1c水平与性别及年龄的关系。方法选取云南汉族居民14267名,检测HbA_1c、FPG、红细胞分布宽度(RDW)水平。将所有研究对象分为HbA_1c<6. 5%组和HbA_1c≥6. 5%组,比较不同性别间HbA_1c水平;再将研究对象按年龄分为≤25岁、26~35岁、36~45岁、46~55岁、56~65岁、66~75岁、>75岁共7组,比较各年龄组HbA_1c水平差异及随年龄变化趋势。Pearson相关分析HbA_1c与FPG和RDW的相关性。Logistic二元回归分析HbA_1c的影响因素。结果云南玉溪地区汉族男性和女性居民HbA_1c水平均高于其他平原地区居民。HbA_1c<6. 5%组中男性、女性HbA_1c水平均随年龄增长而升高,≤55岁时,男性HbA_1c水平均高于女性(P<0. 05);>55岁时,男性和女性HbA_1c水平差异无统计学意义。在HbA_1c≥6. 5%组中,≤55岁时,男性和女性HbA_1c水平均随年龄增高而降低,且男性高于女性(P<0. 05);>55岁时,男性和女性HbA_1c差异无统计学意义。Pearson相关性分析显示,HbA_1c≥6. 5%组FPG与HbA_1c的相关系数高于HbA_1c<6. 5%组(P<0. 05);HbA_1c<6. 5%组RDW与HbA_1c不相关,而在HbA_1c≥6. 5%组相关性增加。Logistic二元回归分析结果显示,FPG和RDW均为HbA_1c异常的影响因素。结论玉溪地区汉族居民HbA_1c水平高于平原地区。性别和年龄是HbA_1c水平的可能影响因素。HbA_1c异常时,对FPG代表性较好,且RDW及FPG均为HbA_1c异常的影响因素。 Objective To investigate the relationship between glycosylated hemoglobin(HbA1c)level and sex and age in Yunnan Han population.Methods A total of 14267 residents in Han residents in Yuxi,Yunnan completed the detection of HbA1c,FPG,and red blood cell distribution width(RDW).The difference in HbA1c between men and women was compared by glucose level stratification(HbA1c<6.5%and≥6.5%).Then,the subjects were divided into 7 groups according to age(≤25,26~35,36~45,46~55,56~65,66~75,>75 years old).The differences in HbA1c levels between age groups and their trends with age were analyzed.Pearson correlation was used to analyze the correlation between HbA1c and FPG and RDW.Logistic binary regression was used to analyze the influencing factors of HbA1c.Results The HbA1c levels of Yunnan Han residents in both men and women were higher than those living in plains.In the population with HbA1c<6.5%,HbA1c levels of men and women increased with age;at≤55 years old,HbA1c in men was higher than women(P<0.05);at>55 years old,there was no significant difference in HbA1c levels between men and women.In the population with HbA1c≥6.5%,when≤55 years old,HbA1c of men and women decreased with age,with more higher level in men(P<0.05);when>55 years old,the differences of HbA1c between men and women had no statistically significant.Pearson correlation analysis showed that the correlation coefficient between FPG and HbA1c in HbA1c≥6.5%group was higher than that in HbA1c<6.5%group(P<0.05).RDW was not associated with HbA1c in the HbA1c<6.5%group;the correlation between the two was increased in group with HbA1c≥6.5%.Logistic binary regression analysis showed that both FPG and RDW were the risk factors for HbA1c abnormalities.Conclusion The level of HbA1c in Han residents in Yuxi,Yunnan is higher than those living in plain regions.Gender and age are possible factors that influence HbA1c levels.Both RDW and FPG are risk factors for HbA1c abnormalities.
