摘要
目的探讨血清载脂蛋白A5(ApoA5)与T2DM及颈动脉粥样硬化(CAS)的关系。方法选取2016年2月至2017年4月于我院内分泌科住院的T2DM患者132例,分为T2DM不合并CAS组(NAS)52例及T2DM合并CAS组(CAS)80例,另选取38名健康体检者作为对照组(NC)。ELISA测定血清ApoA5浓度。结果CAS、NAS、NC组血清ApoA5浓度依次增加[(61. 63±20. 34)vs(79. 04±22. 08)vs(103. 44±21. 40)μg/L,P<0. 01]。Pearson相关分析显示,血清ApoA5与HDL-C呈正相关(r=0. 524,P<0. 01),与TG、BMI、HbA_1c、FIns、FPG、HOMA-IR呈负相关(r=-0. 726、-0. 270、-0. 478、-0. 397、-0. 287、-0. 331,P<0. 01)。与性别、年龄、病程、SBP、DBP、TC、LDL-C不相关(P>0. 05)。多元线性逐步回归结果显示,TG、HDL-C是血清ApoA5的影响因素(β=-18. 580、16. 456,P<0. 01)。结论 T2DM患者ApoA5浓度较正常人低,CAS组ApoA5浓度低于NAS组,提示ApoA5可能是T2DM的保护因素,且与T2DM合并CAS相关,可能成为T2DM大血管病变的潜在治疗靶点。
Objective To investigate the relationship between serum apolipoprotein A5(ApoA5)and carotid atherosclerosis(CAS)in patients with type 2 diabetes(T2 DM).Methods A total of 52 T2 DM patients without CAS(NAS group),80 T2 DM combined with CAS(CAS group)and 38 healthy control subjects(NC group)were recruited in this study.Serum ApoA5 was measured by ELISA.Results The levels of ApoA5 in CAS group and NAS group were lower than that in NC group((61.63±20.34)vs(79.04±22.08)vs(103.44±21.40)μg/L,P<0.01)Serum ApoA5 concentration in CAS group was lower than that in NAS group((61.63±20.34)vs(79.04±22.08)μg/L,P<0.01)Pearson correlation analysis showed that serum apoA5 was negatively correlated with TG,BMI,HbA1c,FIns,FPG and HOMA-IR(r=-0.726,-0.270,-0.478,-0.397,-0.287,-0.331,P<0.01),positively correlated with HDL-C(r=0.524,P<0.01),and no correlation with sex,age,course of disease,SBP,DBP,TC and LDL-C(P>0.05).Multiple linear stepwise regression showed that TG and HDL-C were independent influencing factors of serum ApoA5(β=-18.580,16.456,P<0.01).Conclusion ApoA5 may be a protective factor for T2 DM and is associated with CAS,which may become a potential therapeutic target for T2 DM macroangiopathy.
