氨甲环酸在体外循环中对血液的保护作用

摘要 目的总结氨甲环酸在体外循环中对血液保护作用的临床经验。方法 2007年10月至2008年12月江西省赣州市人民医院选择100例心脏病人随机分A、B两组各50例。A组应用氨甲环酸,B组不用。A组于体外循环结束前30min加入,B组则加等量生理盐水。测定停体外循环前和后10min及停体外循环后2h的血小板计数、手术围手术期出血量和输血情况。结果转流后B组血小板计数明显低于转流前且明显低于A组,出血量和输血量B组明显高于A组。结论体外循环中应用氨甲环酸除明显抑制纤溶系统激活外,能有效保护血小板,明显减少术后出血和输血量。 Objective The present study was designed to summarize the clinical experience in hemostatic effect of tranexamic acid in cardiopulmonary bypass.Methods 100 patients of heart disease were randomized into A,B groups of all 50 patients.In A group,application of tranexamic acid,In B group.do not.A group 50 patients received tranexamic acid on 30 minutes before the end of CPB,B group was plus normal saline.Measured the platelet count,volume of blood loss and blood transfusion case before and stopped 10 minutes after cardiopulmonary bypass,and stopped at 2 hours after cardiopulmonary bypass.Results The platelet count of Group B after CPB was significantly lower than pre-CPB and was significantly lower than group A,the count blood loss and blood transfusion of B Group was significantly higher than A Group.Conclusion The application of tranexamic acid in cardiopulmonary bypass can significantly inhibited the fibrinolytic system activation,inter alia,can effectively protect platelets,significantly reduced the amount of postoperative bleed loss and blood transfusion.
