摘要
目的评价左手控制技术在胃癌手术中的应用效果。方法对2000年以来河南省肿瘤医院普外科应用左手控制技术行Koche,切口,清扫淋巴结及控制出血的临床资料进行分析。结果 120例均顺利完成手术,手术时间明显缩短,出血量明显减少。发生胃右动脉出血2例,胃结干撕裂2例,均经左手控制后成功止血。2例在行第12组淋巴结清扫时门静脉出血,左手控制后缝扎止血成功。结论胃癌切除术中应用左手控制技术可明显缩短手术操作时间,减少出血量,增加手术的安全性和彻底性。
objective To investigate the value of left hand control in the gastric cancer resection.Methods By Kocher' s incision,put the left index finger and middle finger into the lesser omentum,and put the thumb in front of hepatoduodenal ligament,use scissors or electric knife or ultrasound knife to separate liver adipose tissue and lymph nodes,left hand moved to the inside with the operation,and removed article 13,12,8,7,9 lymph nodes.Control the bleeding with the left hand.Results All 120 patients were successfully completed the operations,the time and the amount of bleeding were shortened significantly reduced,2 patients had right gastric artery bleeding and 2 cases had gastric colic artery dilaceration,all of the bleeding had been successful controlled by the left hand;2 patients had portal vein bleeding when dissected the twelfth nodes,and had been successfully controlled under the left control.Conclusion Application of the left hand control technology in the gastric resection can shorten the operation time,reduce the bleeding,increase surgical safety and thoroughness.
