CO_2气腹对重症腹腔炎大鼠抗炎性因子的影响

摘要 目的探讨CO_2气腹对重症腹腔炎大鼠代偿性抗炎反应的影响。方法雄性SD大鼠50只,随机分成5组,每组10只。I组:开腹组;II组:气腹组;Ⅲ组:重症腹腔炎症组;IV组:气腹对照组;V组:开腹对照组。按Wic1hterman的盲肠结扎穿刺法制作大鼠重症腹腔炎模型,进行开腹及气腹处理。分别于术前,术后8h、24h、48h、72h抽外周血测定白介素-4(IL-4)、IL-10。结果与IV组、V组相比,I组、I组、Ⅲ组术前,术后4个时间点IL-4、IL-10均显著升高(P<0.01)。与I组相比,II组术后4个时间点IL-4、IL-10显著降低(P<0.01)。与Ⅲ组相比,I组术后4个时间点IL-4、IL-10均显著升高(P<0.01);Ⅱ组术后4个时间点IL-4、IL-10均升高,但差异无统计学意义(P>0.05)。结论在重症腹腔炎状态下,CO_2气腹缓解术后IL-4、IL-10升高的程度,较开腹明显减轻了术后代偿性抗炎反应。 Objective To study the effects of CO_2 pneumoperitoneum on the compensatoty anti-inflammatory response on Severe abdominal cavity inflammation rats.Methods 50 male SD rats,divided into 5 group randomly,10 for each group.Groupl:laparotomy;Group Ⅱ:pneumoperitoneum;Group Ⅲ:severe abdominal inflamm- ation;Group Ⅳ:pneumoperitoneum control;Group V:laparotomy control.The rat model of severe abdominal inflammation was according to cecal ligation and puncture(CLP) as Wichterman.The Severe abdominal cavity inflammation rats,were laparotomy or treated with CO_2 pneumoperitoneum after cecalling ligation and punctureing(CLP) 16 hours.The blood samples were gathered for examing IL-4,IL-10 before they were treated and after they were treated about 8 hours,24 hours,48 hours,72 hours.Results The experimental results were as follow:In Group Ⅰ,Grou Ⅱ,Group Ⅲ,serum IL-4,IL-10 were significantly higher than in Group Ⅳ and Group Ⅴ before and after the were treated(P<0.01).Serum IL-4,IL-10 in Group Ⅱ were significantly lower than that in Groupl in postoperative(P <0.01).Serum IL-4JL-10 in Groupl were significantly higher than that in Group Ⅲ in postoper ative(P <0.01);they were higher in Group Ⅱ than in Group Ⅲ in postoperative,but there was no significant statistical difference(P >0.05).Conclusion In a state of Severe abdominal cavity inflammation,CO_2 pneumoperitoneum could reduccompensatory anti-inflammatory response,compareing with operation.
