全自动旋切刀治疗乳腺肿物107例临床体会

Clinical experience of minimally invasive mammotome biopsy for breast tumors

目的总结乳腺微创旋切术在乳腺肿瘤诊断及治疗中的应用体会。方法 2012年5月至2013年11月沈阳市第四人民医院对107例乳房肿瘤施行超声引导下乳房微创旋切手术结果 106例均顺利完成手术,1例手术失败,立即中转开刀切除肿物。手术时间15~50 min,平均23.2 min。术后局部瘀斑24例,2周内自然吸收。局部血肿2例,血肿直径2~4 cm,平均3.0 cm,均无需穿刺抽血肿治疗,无开放手术止血及清除血肿病例。结论全自动乳腺旋切刀手术是一种真空辅助的微创切除手术,能够在超声引导下对各种可疑病灶进行穿刺切除活检。在熟练掌握该项技术的基础上可实现缩短手术时间,减小手术切口,准确定位,以及有效美观的治疗效果。

Objective To summarize the value of ultrasound-guidedmammotome in the diagnosis and therapy of breasttumor.MethodsFrom May 2012 to November 2013, a total of 107 cases diagnosed with breast tumor by Ultrasoundguided breast minimally invasive mammotome surgery.Results106 cases were successfully completed surgery,operation failure, immediately transfer to surgical resection of the mass. Operation time 15 to 50 min, an average of 23. 2min. 24 cases of postoperative local ecchymosis in 2 weeks, natural absorption. Local hematoma in 2 cases, hematoma is2 ~ 4 cm in diameter, an average of 3.0 cm, all without puncture pumping haematoma treatment, no open surgeryhemostatic and removal of hematoma cases.ConclusionUltrasound-guidedmammotome is a minimally invasiveprocedure that can safely and reliably diagnose and remove breast tumor, particularly multiple tumors and impalpablelesions. The cosmetic results of the operation are satisfying.
