某医院糖尿病患者手术切口感染影响因素Logistic回归分析

Logistic regression analysis of influencing factors of surgical incision infection in diabetic patients

目的研究糖尿病患者术后切口感染的影响因素。方法回顾调查2014年1月-2018年6月期间在该院行手术治疗的556例糖尿病患者,分为切口感染组和无切口感染组,并对影响患者术后切口感染的相关因素进行Logistic回归模型分析。结果单因素分析显示,糖尿病患者术后切口感染与BMI、合并症、创伤性治疗、备皮至手术时间、手术时限、切口类型等多种因素有关;Logistic多因素分析结果提示,切口类型(Ⅲ类)、糖化血红蛋白>7%、手术时间≥70 min、血清白蛋白<30 g/L等均是导致糖尿病患者术后切口感染的独立危险因素,术中预防使用抗菌药物是糖尿病患者术后切口感染的独立保护因素。结论糖尿病患者术后切口感染与多种因素有关,临床应针对切口感染的影响因素,采取综合有效的预防措施,降低糖尿病患者术后切口感染发生率。

Objective To study the influencing factors of postoperative incision infection in diabetic patients.Methods A retrospective survey of 556 patients with diabetes who underwent surgical treatment in the hospital from January 2014 to June 2018 was conducted.According to whether incision infection occurred or not,the subjects were divided into incision infection group and non-incision infection group.The clinical data of each patient were retrospectively investigated and gradually recorded,and the related factors affecting postoperative incision infection in diabetic patients were analyzed by Logistic regression model.Results Univariate analysis showed that the postoperative incision infection in diabetic patients was associated with BMI,complications,traumatic treatment,and time from skin preparation to operation.Logistic multivariate analysis suggested that incision type(Class III),glycosylated hemoglobin>7%,operative time≥70 min,serum albumin<30 g/L,etc.were all independent risk factors for postoperative incision infection in diabetic patients.Preventive use of antibiotics during surgery was an independent protective factor for postoperative incision infection in diabetic patients.Conclusion Postoperative incision infection in diabetic patients is related to a variety of factors.Clinically,comprehensive and effective preventive measures should be taken to reduce the incidence of postoperative incision infection in diabetic patients according to the influencing factors of incision infection.
