心房颤动射频消融术后复发与非复发患者中性粒细胞与淋巴细胞比值的比较

Comparison of neutrophil/lymphocyte ratio in patients with recurrence and non-recurrence after radiofrequency ablation of atrial fibrillation

目的探讨外周血中性粒细胞/淋巴细胞比值(NLR)对心房颤动(简称房颤)患者射频消融术后复发的预测价值。方法选取2016年9月至2017年9月行射频消融术的房颤患者154例。依据随访结果将入组患者分为复发组及未复发组,比较两组患者的一般临床资料及房颤病程时间、射血分数、左房内径、白细胞及NLR等指标。结果复发组的术前NLR水平显著高于未复发组[3.3±3.7vs 1.8±0.7,P<0.01],复发组左房内径亦高于未复发组[(43.0±5.9)mm vs(37.9±6.1)mm]。多因素Logistic回归分析结果表明,房颤消融术前患者的NLR水平可独立预测术后复发(OR=3.24,95%CI:1.82～5.77,P<0.01)。结论行射频消融术的房颤患者其术前NLR水平与术后房颤复发密切相关,且高NLR水平是术后房颤复发的独立危险因素。

Objective To investigate the predictive value of peripheral neutrophil-to-lymphocyte ratio(NLR)for recurrence after radiofrequency ablation in patients with atrial fibrillation(AF).Methods This study enrolled 154 AF patients who underwent radiofrequency ablation.According to the follow-up results,the patients were divided into recurrent group and non-recurred group.The general clinical data and duration of atrial fibrillation,ejection fraction,left atrial diameter,white blood cell and NLR were compared.Results The NLR value was significantly higher in patients with AF recurrence than that in patients without AF recurrence(3.3±3.7 vs.1.8±0.7,P<0.01).The left atrial diameter was also significantly higher in patients with AF recurrence than in patients without AF recurrence[(43.0±5.9)mm vs(37.9±6.1)mm,P<0.01].Multivariate logistic regression result indicated that preoperative NLR could independently predict the recurrence of AF after ablation(OR=3.24,95%CI:1.82-5.77,P<0.01).Conclusion Preoperative NLR is closely associated with the recurrence of AF after radiofrequency ablation,and high NLR vale is an independent predictor for AF recurrence after ablation.
