基于精益管理的医院手术业务流程现况研究

Study on the current situation of hospital operation process based on lean management

目的:了解医院手术业务流程现况问题,为制定科学、有效的流程优化与改进方案提供理论支撑。方法:通过问卷调查及选取60名手术患者实施全流程追踪,运用精益生产的管理工具系统分析手术业务流程现况,查找瓶颈问题及"浪费"环节并分析存在原因。结果:手术患者在手术运行流程内的周期时间均值为9.5小时,纯手术时间为6.49小时,非手术时间为3.01小时;其中增值时间均值为1.73小时,非增值时间为1.28小时,增值时间与手术非增值时间占比为1∶0.73。结论:在手术业务流程中存在影响手术效率的"浪费"环节及"非增值"时间制约了手术的高效运行,应制定系统化的改进方案、借鉴精益生产的管理工具实施流程优化,以提高手术运行效率。

Objectives: To analyze the present situation of operation process in hospital, find out problems and to provide theoretical support for scientific and effective process optimization and improvement. Methods: A total of 60 patients were investigated by questionnaire and 60 patients were selected to follow up the whole procedure. The management tools of lean production were used to analyze the present situation of the operation business process,to find the bottleneck question and the link of waste and to analyze the reason of the problem. Results: The mean weekly time was 9.5 hours, the pure operation time was 6.49 hours, the non-operative time was 3.01 hours, and the value added time was 1.73 hours. The non-value-added time was 1.28 hours, and the ratio of value-added time to operative non-value-added time was 1: 0.73. Conclusions: There are waste links and non-value-added time that affect the efficiency of the operation in the operation process and needs to make a systematic plan to improve the operation efficiency, and use the management tools of lean production for reference to implement the optimization of the operation process.
