炎复康阴道冲洗预防宫颈癌放疗阴道损伤的疗效

Efficacy of vaginal irrigation with Yanfukang to prevent radiotherapy injury to the vagina in cervical cancer

目的探讨炎复康洗液阴道冲洗在宫颈癌患者同步放化疗中的临床观察及预防放射性阴道损伤的方法。方法选取2016年6月至2018年6月间陕西中医药大学第二附属医院收治的100例非手术宫颈癌患者,采用随机数表法分为观察组和对照组,每组50例。观察组患者在同步放化疗基础上采用炎复康洗液阴道冲洗,对照组患者在同步放化疗基础上采用1∶1 000新洁尔灭阴道冲洗。两组患者阴道冲洗均坚持至治疗结束后3个月。观察两组患者的病灶局部控制率、治疗期间继发感染率及治疗结束后随即、治疗后1个月、2个月和3个月阴道炎和阴道粘连情况。结果观察组患者治疗有效率为88. 0%,对照组患者为86. 0%,差异无统计学意义(P> 0. 05)。观察组患者治疗期间继发感染发生率为2. 0%(1例),对照组患者为20. 0%(10例),差异有统计学意义(P <0. 05)。观察组患者治疗结束随即、治疗后1个月、2个月和3个月时的阴道炎和阴道粘连发生情况均少于对照组患者,差异均有统计学意义(均P <0. 05)。结论采用炎复康洗液阴道冲洗不影响临床疗效,可有效控制治疗期间继发感染率,预防放射性阴道损伤发生率,值得临床推广。

Objective To explore the method of preventing radiotherapy induced vaginal injury by clinical observation of Yanfukang lotion washing vagina in concurrent radiotherapy and chemotherapy for cervical cancer.Methods A total of 100 patients receiving non-surgical management of cervical cancer at Second Affiliated Hospital of Shaanxi University of Traditional Chinese Medicine were randomly divided into an observation group and a control group with 50 patients in each group from June 2016 to June 2018.Both groups were treated with concurrent radiotherapy and chemotherapy.The observation group was given vaginal irrigation with Yanfukang lotion,while the control group was given vaginal irrigation with 1∶1 000 bromogeramine.The vaginal irrigation of both groups lasted until 3 months after the end of treatment.Local control rate,secondary infection rate during treatment,and immediate after treatment,and the the incidence of vaginitis and vaginal adhesions at 1,2 and 3 months after treatment were observed.Results The incidence of secondary infection was 88.0%in the observation group and 86.0%in the control group(P>0.05).The incidence of vaginitis and vaginal adhesion was 2.0%(1 patient)in the observation group which was lower than 20.0%(10 patients)of the control group(P<0.05).Conclusion Vaginal irrigation with Yanfukang lotion does not affect the clinical efficacy.It can effectively control the secondary infection rate and prevent the incidence of radiation vaginal injury during treatment,and improve the quality of life.It is worthy of clinical promotion.
