紫杉醇脂质体联合顺铂与吉西他滨联合顺铂一线治疗晚期肺鳞癌患者的疗效比较

Comparison of paclitaxel liposome versus cisplatin and gemcitabine in the treatment of advanced lung squamous cell carcinoma

目的探讨注射用紫杉醇脂质体联合顺铂(LP方案)与吉西他滨联合顺铂(GP方案)一线治疗晚期肺鳞癌患者的临床疗效和不良反应。方法选取2010年7月至2016年7月间山东省肿瘤医院收治的200例晚期肺鳞癌患者,根据治疗方法不同分为LP组和GP组,每组100例。LP组患者采用注射用紫杉醇脂质体联合顺铂治疗,GP组患者采用吉西他滨联合顺铂一线化疗治疗,比较两组患者的临床疗效和不良反应。结果 LP组患者有效率(RR)为39. 0%,疾病控制率(DCR)为88. 0%,GP组患者RR为43. 0%,DCR为87. 0%; LP组患者中位无进展生存时间(OS)为5. 6个月,中位总生存时间(PFS)为14. 1个月; GP组患者PFS为6. 1个月,OS为13. 5个月。两组比较,差异均无统计学意义(均P> 0. 05)。GP组患者的白细胞减少、血小板减少和胃肠道反应较LP组更明显,差异均有统计学意义(均P <0. 05)。结论 LP和GP两种方案治疗晚期肺鳞状细胞癌患者的临床疗效相似,LP方案不良反应较轻。

Objective To explore the clinical efficacy and adverse reactions of first-line paclitaxel liposome combined with cisplatin(LP regimen)versus gemcitabine combined with cisplatin(GP regimen)in the treatment of advanced lung squamous cell carcinoma.Methods Two hundred patients with advanced lung squamous cell carcinoma who were admitted to Shandong Provincial Cancer Hospital from July 2010 to July 2016 were selected.According to the therapies,the patients were divided into a LP group and a GP group with 100 patients in each group.The LP group received paclitaxel liposome injection combined with cisplatin and the GP group received first line gemcitabine plus cisplatin and their efficacy and adverse reactions were compared.Results The relative risk was 39.0%and the disease control rate was 88.0%in LP group and it was 43.0%and 87.0%,respectively for the GP group.The median median progression-free survival(PFS)was 5.6 months and overall median survival was 14.1 months and the median PFS and overall median survival was 6.1 months and 13.5 months(P>0.05).Compared with the the GP group,the LP group was lighter in myelosuppression and gastrointestinal reactions(all P<0.05).Conclusion The efficacy of the LP regimen in the treatment of advanced lung squamous cell carcinoma is similar to that of the GP regimen,but the LP regimen has less adverse reactions in myelosuppression and gastrointestinal reactions.
