康复护理对食管癌患者术后肺部感染的效果

Efficacy of rehabilitation nursing for postoperative pulmonary infection in patients with esophageal cancer

目的探讨康复护理对食管癌患者术后肺部感染的效果。方法选取2016年7月至2017年7月间湖北省宜昌市夷陵医院收治的118例行食管癌根治术患者,将采用康复护理的59例患者纳入研究组,采用常规护理的59例患者纳入对照组。比较两组患者术后肺部感染率和护理质量,并分析食管癌患者术后肺部感染的危险因素。结果研究组患者术后肺部感染发生率为11. 9%,低于对照组患者的33. 9%,差异有统计学意义(P <0. 05)。肺部感染患者是否微创手术,是否管状胃、手术时间、淋巴结三野清扫、喉返神经损伤和弓下吻合与未感染患者分别比较,差异均有统计学意义(均P <0. 05)。其中,淋巴结三野清扫、喉返神经损伤、弓下吻合和手术时间较长均是食管癌患者术后发生肺部感染的独立危险因素,差异均有统计学意义(均P <0. 05)。研究组患者的业务水平、护理态度、护理技巧和健康宣教得分均高于对照组患者,差异均有统计学意义(均P <0. 05)。结论食管癌患者术后肺部感染的影响因素包括弓下吻合、淋巴结三野清扫、喉返神经损伤和手术时间,对食管癌术后患者运用康复护理,能够减少肺部感染的发生率,有着较好的临床效果。

Objective To explore the efficacy of rehabilitation nursing for postoperative pulmonary infection in patients with esophageal.Methods From July 2016 to July 2017,118 patients undergoing esophageal cancer resection were selected at Yiling Hospital.Using the lottery method,they were divided into a study group and a control group with 59 patients in each group.The study group was given rehabilitation nursing and the control group was given conventional nursing.Postoperative pulmonary infection rate and nursing quality were compared between the two groups.The risk factors influencing postoperative pulmonary infection in patients with esophageal cancer were analyzed.Results The incidence of postoperative pulmonary infection was 11.8%in the study group which was lower than 33.9%of the control group(P<0.05).There was significant difference in operative time,lymph node dissection,recurrent laryngeal nerve injury,and subarcual anastomosis between infected patients and uninfected patients(all P<0.05).Lymph node dissection,recurrent laryngeal nerve injury,subarcual anastomosis and long operative time are independent risk factors for pulmonary infection after esophageal cancer(all P<0.05).The scores of nursing attitude,nursing skill,and health education were higher in the study group than in the control group(all P<0.05).Conclusion Factors influencing postoperative lung infections in elderly patients with esophageal cancer surgery include bow under anastomosis,lymph node cleaning,recurrent laryngeal nerve injury and operation time.The rehabilitation nursing can help to reduce the incidence of postoperative pulmonary infection and has a relatively good clinical efficacy in patients with esophageal cancer.
