肿瘤患者经外周静脉置入中心静脉导管化疗的心理状况及其影响因素

Psychological status and its influencing factors in patients with cancer undergoing PICC placement for chemotherapy

目的探讨肿瘤患者经外周静脉置入中心静脉导管(PICC)置管化疗期间心理状态及其影响因素。方法选取2016年9月至2018年9月间武汉市第一医院收治的80例肿瘤患者,采用问卷法对所有患者的心理状态进行调查,包括抑郁自评量表(SDS)和焦虑自评量表(SAS),探讨影响患者焦虑和抑郁的危险因素。结果单因素分析显示,肿瘤PICC置管化疗患者焦虑、抑郁总分在性别、职业状况、家庭收入情况、医药费用来源、病理分期、并发症发生情况以及对疾病是否恐惧等方面的比较,差异均有统计学意义(均P <0. 05)。Logistic多因素回归分析结果显示,肿瘤PICC置管化疗患者焦虑情绪与家庭年收入、对疾病是否恐惧、并发症发生情况、病理分期、性别、职业状况以及医药费用来源有关,差异均有统计学意义(均P <0. 05)。肿瘤PICC置管化疗患者抑郁情绪与病理分期、性别、职业状况、医药费用来源、并发症发生情况以及对疾病是否恐惧有关,差异均有统计学意义(均P <0. 05)。结论肿瘤PICC置管化疗的患者均存在一定程度的抑郁、焦躁等负面心理状况,影响因素主要有家庭年收入、对疾病是否恐惧、病理分期、性别、职业状况、医药费用来源以及并发症发生情况。

Objective To investigate the psychological status and its influencing factors in patients with cancer undergoing peripherally inserted central venous catheters(PICC)placement for chemotherapy,Methods In this study,the psychological state of 80 patients with cancer was investigated by questionnaire from September 2016 to September 2018 at Wuhan First Hospital.Self-rating depression scale(SDS)and self-rating anxiety scale(SAS)were used to evaluate the risk factors affecting anxiety and depression.Results Univariate analysis showed that there were significant differences in the distribution of gender,occupational status,family income,source of medical expenses,pathological stage,complications,and fear of disease among the patients with different overall scores of SDS and SAS(all P<0.05).Logistic multivariate analysis showed anxiety was associated with family annual income,fear of disease,pathological staging,gender,occupational status and medical expenses in patients with cancer undergoing PICC placement for chemotherapy(all P<0.05).Depression was associated with pathological staging,gender,occupational status,source of medical expenses,complications,and fear of disease in patients with cancer undergoing PICC placement for chemotherapy(all P<0.05).Conclusion Patients with with cancer undergoing PICC placement for chemotherapy have a certain degree of negative psychological status such as depression and anxiety which was mainly influenced by factors such as family annual income,fear of disease,pathological stage,gender,occupational status,source of medical expenses,and complications.
