绝经综合征178例临床治疗疗效观察

Clinical observation on the therapeutic effect of 178 women with menopausal syndrome

目的通过临床各种治疗方案比较,寻找最佳治疗方案,预防和减轻绝经前后妇女的临床症状,以使她们平稳过渡此期,提高生活质量,从而维护家庭和社会的稳定。方法选取2009年1月1日至2011年6月30日在我院妇女保健科门诊就诊的178例围绝经期妇女,在进行全面的体格检查,确定无用药禁忌后随机将她们分为3组,每一组用药均在6个疗程以上,第一组60人单用醋酸甲羟孕酮复合胶囊(商用名妇复春),第二组59人服用妇复春时加用维生素C及浓缩六味地黄丸,第三组59人,服用妇复春、维生素C及浓缩六味地黄丸的同时每周进行盆底肌电刺激理疗3次。结果所有患者全身症状都有改善,用药6个疗程后所有症状都得到了控制,但年龄偏大及月经紊乱或绝经者单用妇复春治疗效果相对较慢。而并发有阴道壁松驰或张力性尿失禁者须同时加做盆底电刺激治疗后明显增强了对膀胱及阴道的括约功能[1],张力性尿失禁自治疗第三个疗程后再无发生,自感生活质量提高。结论妇复春能有效治疗围绝经期综合征,合并有尿道综合征者辅用维生素C及六味地黄丸疗效更好,而对于并发有阴道壁松驰、张力性尿失禁者加做盆底肌电刺激具有独到的效果。

Objective We aim to identify the optimal treatment for preventing or relieving the symptoms by comparing different treatment.It helps women make the transition smoothly,improve the quality of life,and maintain the family and social stability.Methods We recruited 178 perimenopausal women in our out-patient clinic from Jan 1 2009 to Jun 30 2011.After physical examination and exclusion of those with-contraindications,we randomly separated our subjects into three groups.And all groups should be under treatment for six courses of treatment.60 subjects in the first group were treated by Medroxyprogesterone Acetate Complex Capsules(Fufuchun).59 subjects in the second group were treated by Fufuchun,Vitamin C and Concentrated Liuwei Dihuang Pills.In the third group,59 subjects were treated by Fufuchun,Vitamin C and Concentrated Liuwei Dihuang Pills.Meanwhile,women in third group received pelvic floor muscles electrical stimulation therapy three times a week.Results Systemic symptoms has significantly relieved in all patients in this study.After six courses of treatment,all symptoms have been under control.However,the recovery process of older and menstrual disorders or postmenopausal women under only Fufuchun treatment is relatively slow.The functions of sphincter in bladder and vagina were significantly improved in patients with vaginal wall relaxation or stress incontinence in third group.In addition,patients with stress incontinence did not relapse after three courses of treatment and the quality of life was enhanced.Conclusions Fufuchun can effectively treat the menopausal syndrome.The treatment with Fufuchun,vitamin C and Concentrated Liuwei Dihuang Pills showed better curative effect for patients with urethral syndrome.In addition,the treatment auxiliary with pelvic floor muscles electrical stimulation therapy had the unique effect for patients with vaginal wall relaxation or stress incontinence.
