我院Ⅰ类切口围手术期预防应用抗菌药物调查分析

目的探讨临床药师干预对围手术期预防用抗菌药物使用的有效性。方法以2009年-2011年连续三年普外科Ⅰ类切口手术住院病历300份,对抗菌药物使用率、平均用药天数、预防用药品种数及合理性进行统计,分析实施干预前后抗菌药物使用情况。结果干预后甲状腺手术、乳腺手术、腹外疝修补术围手术期预防用抗菌药物使用率显著下降,由干预前的100%分别下降为干预后的29.4%、34.3%、38.7%;抗菌药物品种的使用合理性明显提高,平均用药天数由干预前的6.5d、6.5d、6.1d分别下降到干预后的1.6d、1.8d、1.9d。结论临床药师通过参与抗菌药物临床应用管理,使抗菌药物的不合理使用现象明显改善,对规范应用抗菌药物有很大作用。

Objective To study the effectiveness of the clinical pharmacist’s interfering with the preventive use of perioperative antibacterials.Method To make statistical of 100 medical records copies of general surgery incision operation inpatient in the three consecutive years from 2009 to 2011,the use rate of antibacterial,the average duration of medication,the varieties and rationality of the prevention drug,and the analyze the use of antibacterial drugs before and after the implementation of interfering.Result After the intervention,the antibacterials use rate decreased significanty of the thyroid operation,breast operation,abdominal external hernia repair operation during the perioperative period.From 100%before intervention to 29.4%,34.3%,38.7%.The use rationality obviously increased of the antibacterial drugs use.The average number of drug use days from the 6.5d,6.5d,6.1d before the intervention to the 1.6d,1.8d,1.9d after.Conclusion The clinical pharmacists improved the phenomenon of the unreasonable use of antibacterial by participating in management of clinical application of antibacterial and also play a significant role in standardizing the application of antibacterial drugs.
