摘要 目的探讨血超敏C反应蛋白(Hs-CRP)和胱抑素C(CysC)水平的变化与T2DM诊断的临床意义。方法将T2DM患者以24小时尿白蛋白排泄率(UAER)水平予分为单纯的T2DM组(Ⅰ组),早期DN组(Ⅱ组),分别测定各组间患者血Hs-CRP、CysC、尿素氮(BUN)、肌酐(Cr)以及UAER的浓度,并且与对照组(Ⅲ组)进行比较。结果单纯的T2DM组、DN早期组中的血Hs-CRP、CysC水平明显高于对照组,统计学分析有差异(P【005)。DN早期组中的血Hs-CRP、CysC水平明星高于单纯的T2DM组(P【005)。随着DN病情的进展,血Hs-CRP、CysC水平显著升高(P【0.05)。结论在DN的发生发展中,血Hs-CRP、CysC浓度变化水平与DN患者病情成正比,可作为诊断DN的早期敏感、可靠的常规性指标。因此,在DN早期,血Hs-CRP、CysC可为诊断及监测病变进展提供一定的临床价值。 Objective To investigate the changes in blood hypersensitive C-reactive protein as well as the Cys C levels and the clinical significance of early diagnosis in patients with the type 2 diabetes Diabetic nephropathy.Methods The type 2 diabetes patients are divided into simple diabetic group( group Ⅱ) ,early diabetic ne-phropathy group( group Ⅲ) by 24-hour urinary albumin excretion rate level.The concentration of CRP,Cys C,BUN,Cr and UAER in each group are measured respectively and compared with the control group( group Ⅰ) .Results The levels of CRP and Cys C of simple diabetic group and the group of early diabetic DN are significantly higher than the control group.There exit statistical differences( P < 0.05) .The levels of CRP and Cys C of early diabetic DN group is significantly higher than the group of simple diabetic( P < 0.05) .With the diabetic DN condition worsens,the levels of blood CRP and CysC increase distinctively( P < 0.05) .Conclusions During the occurrence and progression of the diabetic DN,we can regard the levels of concentration changes of CRP and Cys C as sensitive and reliable conventional indicators in early diagnosis of diabetic DN.Therefore,early in the diabetic DN,CRP and CysC in blood may provide some clinical value for the diagnosis and monitoring of lesion degree of progress.
