关节镜下自-异体肌腱重建后交叉韧带的临床研究

Clinical Study of Arthroscopic Reconstruction of Posterior Cruciate Ligament with Autograft and Allograft Tendon

目的探讨比较关节镜下自-异体肌腱重建后交叉韧带的临床效果。方法选取本院2012年3月至2015年2月间56例行关节镜下膝关节后交叉韧带重建术的患者,将其分为A组(30例)与B组(26例)。A组采取自体肌腱移植,B组采取自-异体肌腱移植,所有患者均进行12个月至18个月的随访,观察比较两组的手术时间、不良反应发生情况、住院时间及术后的Larson评分、Lysholm评分、IKDC评分(国际膝关节评分委员会)。结果 A组的手术时间长于B组(P<0.05),住院时间短于B组(P<0.05),两组均未出现明显不良反应,两组的Larson评分、Lysholm评分、IKDC评分的相比较,无明显差异(P>0.05)。结论自-异体肌腱移植在膝关节后交叉韧带的重建中具有良好的治疗效果,与自体肌腱移植的疗效无明显差异。

Objective To investigate the clinical effect of arthroscopic reconstruction of posterior cruciate ligament.Methods In our hospital from March 2012 to February 2015,56 patients underwent arthroscopic reconstruction of posterior cruciate ligament,and divided them into group A(30 cases)and group B(26 cases).Group a taken autologous tendon transplantation and auto/allogenic tendon transplantation,all patients were followed up for 12 months to 18 months,observed were compared between the two groups in the operation time,the adverse reaction incidence,hospitalization time and operation after the Larson score,Lysholm score,IKDC score(International Knee Score Board)was adopted in the B group.Results The operation time of group A is longer than B group(P<0.05),duration of hospital stay was shorter in group B(P<0.05),two groups did not appear significantly adverse reactions,two groups of the Larson score,Lysholm score,IKDC score,no obvious difference(P>0.05).Conclusion The autologous tendon allograft has good therapeutic effect in the reconstruction of the posterior cruciate ligament of the knee,and there is no significant difference in the curative effect of autologous tendon transplantation.
