乳腺导管内乳头状瘤导管造影X线特征分析

Mammographic Characteristics of Intraductal Papilloma of Breast

目的对乳腺导管内乳头状瘤的造影X线特征进行分析探讨,并对其诊断价值进行探究。方法对58例乳腺导管内乳头状瘤患者的临床资料进行回顾性分析总结。结果在X线片中测得12例中央区出现大小不一的结节影,其中出现3例粗颗粒样或弧形钙化,4例出现导管影增粗,3例合并出现粗颗粒样及结节弧形钙化并同时伴有导管增粗,2例多脂性乳房中央区出现导管增粗,45例X线片中未发现明显的阳性特征;进行乳导管造影的患者为54例(93.1%),能在不同部位中发现乳导管出现扩张,其中33例增粗大导管中末端出现杯口状截断,21例导管内出现卵圆形或圆形的柱状缺损或充盈,且存在3例患者多发。对4例小导管病变发生情况出现误诊漏诊问题,其中1例乳导管轻度狭窄出现漏诊,2例无明显导管变化,仅见小节影患者误诊为小纤维腺瘤,1例导管内乳头状癌误诊为导管内乳头状瘤。结论采用乳导管造影能准确对乳腺导管内的乳头状肿瘤进行定位于定性,是当前临床上对导管内乳头状瘤进行诊断的可靠影像学方法,临床应用价值不可忽略。

Objective To analyze and discuss the X-ray features of intraductal papilloma of breast,and to explore its diagnostic value.Methods The clinical data of 58 patients with intraductal papilloma of breast were retrospectively analyzed and summarized.Results The X-ray measured the size of the nodules occurred in 12 cases of central area,in which there were 3 cases of coarse granular or curved calcification,4 cases of ductal thickening,3 case with coarse granular and nodular calcification and arc ductal thickening,2 cases of central region of breast fat ductal thickening,45 cases of X-ray film was found in the positive characteristics;of galactography were 54 cases(93.1 %),can be found in different parts in the mammary duct expansion,which increased at the end of 33 cases occurred in the cup like thick tube truncated oval or circular cylindrical or filling defect appears 21 cases of intraductal,and 3 patients had multiple.4 cases of small duct lesions appear misdiagnosis,1 cases of mammary duct mild stenosis of misdiagnosis,2 cases had no obvious change was observed in patients with catheter,section shadow misdiagnosed as small fibroadenoma,1 cases of intraductal papillary carcinoma misdiagnosed as intraductal papilloma.Conclusion The galactography can accurately for intraductal papillary tumor were located in the qualitative,is the current treatment of intraductal papilloma were reliable diagnostic imaging method,clinical application value can not be ignored.
