腹腔镜治疗卵巢囊肿的止血方式分析

Analysis of Hemostatic Methods for Laparoscopic Treatment of Ovarian Cyst

目的了解接受腹腔镜手术方案治疗的卵巢囊肿患者选择止血方式对其临床影响情况。方法分析对象来自本院2013年-2017年期间收治采取腹腔镜下卵巢囊肿剥除术手段治疗患者相关临床资料,从中选取60例,根据患者所接受止血方式差异将其每组30例分为A组与B组,其对应止血方式为电凝止血、镜下缝合止血。对患者术后进行随访以了解不同止血方式对其临床指标影响。结果对患者进行平均时间6个月随访,在术前、术后1个月及术后6个月对以下指标进行检测比较:血清雌二醇(E2)、促卵泡激素(FSH)、促黄体生成素(LH)、卵巢间质血流收缩期峰值(PSV)、卵巢窦卵泡数,相对于治疗前,患者各项指标均出现变化,术后1个月A组数据变化程度明显于B组,差异经统计学软件处理(P<0.05);B组患者术后6个月各项指标所得数据与术前比较无统计学意义(P>0.05)。结论对腹腔镜手术治疗的卵巢囊肿患者选择镜下缝合的止血手段效果突出,同时在术后对患者机体指标情况影响程度明显小于电凝止血手段,值得临床推广。

Objective To investigate the clinical effect of selective hemostasis on ovarian cysts in patients undergoing laparoscopic surgery. Methods To analyze the object from the hospital in 2013-2017 were taken during laparoscopic ovarian cystectomy treatment patients with the clinical data from 60 cases selected, according to the received hemostasis difference with 30 cases in each group is divided into the A group and B group, the corresponding methods of hemostasis for electric coagulation hemostasis, endoscopic suture hemostasis. The patients were followed up to understand the influence of different hemostatic modalities on their clinical parameters. Results The patients were followed up for an average of 6 months. The following indexes were detected before operation, 1 months after operation and 6 months after operation. Serum estradiol(E2) was compared, Follicle stimulating hormone(FSH), Luteinizing hormone(LH) and ovarian interstitial blood flow systolic peak(PSV), Ovarian antral follicle count, relative to treatment prior to treatment, All the indexes of the patients changed, 1 months after operation, the change degree of A in group was obviously higher than that in group B, the difference was statistically treated by software(P<0.05); the data of each index in B group were not statistically significant compared with that before operation(P>0.05). Conclusion The laparoscopic treatment of ovarian cyst patients choose the hemostatic effect means endoscopic suturing of the prominent, while the postoperative body index in patients with the extent of the impact is significantly less than the coagulation method, is worthy of promotion.
