无保护会阴接生法在自然分娩中应用观察

Clinical Observation of No Protection of Perineum Midwives in Natural Labor

目的探讨无保护会阴接生法在自然分娩中的应用效果。方法选取2016年1月至2017年1月期间我院妇产科收治的初产妇500例,均为正常足月妊娠且实施自然分娩方式,按照双盲法将其分成观察组和对照组,每组各250例。给予对照组产妇会阴保护接生法,给予观察组产妇无保护会阴接生法,比较两组产妇的会阴切开与撕裂情况、第二产程时间以及产后会阴疼痛评分。结果观察组会阴完整率高于对照组,会阴切开率低于对照组,有统计学意义(P<0.01),会阴撕裂情况无统计学差异(P>0.05);观察组产妇的第二产程时间、产后会阴疼痛评分明显低于对照组,有统计学差异(P<0.01)。结论对自然分娩产妇实施无保护会阴接生法,能够有效减少产妇的会阴切开,缩短产程时间,减轻术后会阴疼痛程度,有效促进自然分娩的实现。

Objective To investigate clinical efficacy of no protection of perineum midwives in natural labor. Methods During the period from January 2016 to January 2017 in our hospital from 500 cases of primipara, were normal full-term pregnancy and natural childbirth way, in accordance with the double blind method divided into observation group and control group, 250 cases in each group.The control group was given the protection of maternal perineal delivery method, the observation group was given no protection of perineum midwives, compared two groups of maternal perineal incision score and tear, time of the second stage of labor and postpartum perineal pain. Results The intact rate of perineum in observation group was higher than that in control group, and the rate of episiotomy was lower than that of control group,(P<0.01). No significant difference of perineal laceration(P>0.05); Time of the second stage of labor postpartum and perineal pain score of the observation group of were significantly lower than the control group(P<0.01). ConclusionThe implementation of natural delivery of unprotected perineum midwives, can effectively reduce the maternal perineal incision, shorten delivery time, reduce postoperative perineal pain, effectively promote natural childbirth.
