观察无创通气联合氨溴索雾化治疗慢阻肺合并呼吸衰竭的效果

To Observe the Efficacy of Noninvasive Ventilation Combined with Ambroxol Nebulization in the Treatment of COPD with Respiratory Failure

目的探讨无创通气联合氨溴索雾化治疗慢阻肺合并呼吸衰竭的临床效果。方法抽取2016年1月-2017年1月间来我院诊治的慢阻肺合并呼吸衰竭患者86例,随机平均分为两组,分别给予常规治疗(对照组)及无创通气联合氨溴索雾化治疗(观察组),对其治疗效果进行对比分析。结果治疗后观察组的动脉血氧分压(PaO_2),二氧化碳分压(PaCO_2),氧合指数(PaO_2/FiO_2),第1秒用力呼气容积/用力肺活量(FEV_1/FVC),用力呼气容积占预计值百分比(FEV1%)以及呼吸功能恢复时间等各项指标均优于对照组;治疗总有效率观察组为95.3%,对照组为72.1%。结论无创通气联合氨溴索雾化治疗慢阻肺合并呼吸衰竭具有无创伤性、操作简便、临床症状改善明显、血气分析和肺功能恢复好等优势,值得临床推广应用。

Objective To investigate the clinical efficacy of noninvasive ventilation combined with ambroxol nebulization in the treatment of chronic obstructive pulmonary disease(COPD) with respiratory failure. Methods 86 patients with COPD with respiratory failure from January 2016 to January 2017 in our hospital were collected, randomly divided into two groups,respectively given conventional treatment(control group) and noninvasive ventilation combined with ambroxol inhalation(observation group) and the therapeutic effects were compared and analyzed. Results Arterial oxygen pressure(PaO_2), carbon dioxide partial pressure(PaCO_2), oxygenation index(PaO_2/FiO_2),forced expiratory volume in 1 second/forced vital capacity(FEV_1/FVC), forced expiratory volume percentage of predicted value(FEV1%) and respiratory function recovery time and other indicators of the observation group were better than those of the control group, the total effective rate was 95.3% in the observation group and 72.1% in the control group. Conclusion Noninvasive ventilation combined with ambroxol inhalation in the treatment of chronic obstructive pulmonary disease with respiratory failure has many advantages such as non-invasive,simple operation, obvious improvement of clinical symptoms, good blood gas analysis and lung function recovery,and is worthy of clinical application.
