评价自身免疫性甲状腺疾病诊疗中三种自身抗体的临床价值

To Evaluate the Clinical Value of Three Autoantibodies in the Diagnosis and Treatment of Autoimmune Thyroid Disease

目的分析研讨自身免疫性甲状腺疾病(AITD)诊疗中三种自身抗体[抗甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(Tg Ab)、TSH受体抗体(TRAb)]的临床价值。方法随机从我院2015年1月至2016年12月收治的患者中,抽取860例纳入到讨论中,其中包含AITD甲亢430例,非AITD430例,对照组健康人群共120例,测定其血清TPOAb、血清Tg Ab、血清TRAb,以及FT4、FT3指数,并对比分析。结果 TRAb正常标准为:<1.75IU/L,Tg Ab正常标准为<4.11IU/m L,TPOAb正常值标准为<5.61IU/m L。结果证实,AITD甲亢组TPOAb、Tg Ab、TRAb阳性率比其余二组高(P<0.05);对照组TPOAb、Tg Ab、TRAb测定结果与非AITD组对比则无异常(P>0.05)。结论临床可通过测定甲状腺自身抗体指数来判定AITD甲亢疾病,且对疾病诊断鉴别、监测疗效、疾病复发预测等临床意义较大。

Objective To analyze and discuss the clinical value of three autoantibodies(thyroid microsome antibody,thyroglobulin antibody and TSF receptor antibody) in the diagnosis and treatment of autoimmune thyroid disease.Methods randomly from our hospital from January 2015 to December 2016 were selected from 860 cases of patients,into the discussion,including 430 cases of AITD patients with hyperthyroidism,hypothyroidism group AITD 430 cases in the same period of 430 cases with non AITD patients,the control group of 120 healthy individuals were measured serum TPOAb,Tg Ab,the serum TRAb,and comparative analysis.Results The normal standard of TRAb was:<1.75 IU/L,while the Tg Ab was:<4.11 IU/m L,and the TPOAb was:<5.61 IU/m L.The results confirmed that in the AITD hyperthyroidism group the positive rates of TRAb and TPOAb and TGAb are higher than the other two groups(P<0.05);while the TPOAb,Tg Ab,TRAb results with the non AITD group had no abnormal contrast(P> 0.05).Conclusion The clinical value of thyroid autoantibody index can be used to determine AITD disease,and the clinical significance of the disease diagnosis,identification,monitoring efficacy and prediction of disease recurrence is greater.
