探讨喉罩和气管插管全麻在腹腔镜胆囊手术中的应用价值

Objective to Investigate the Value of Laryngeal Mask Airway and Endotracheal Intubation for Laparoscopic Cholecystectomy

目的探讨喉罩全麻和气管插管全麻在腹腔镜胆囊手术中的应用价值。方法将我院2016年5月-2017年5月92例需要进行腹腔镜胆囊手术的患者进行了系统的回顾与分析,将它们分成了两组,每组各46例,A组喉罩全麻,B组气管插管全麻。对两组在手术中、手术后患者的心率、血压等进行认真观察并比较,对并发症的情况也需要统计。结果 A组喉罩全麻在手术治疗前和手术后,心率和血压无明显变化,而且术后拔管、清醒时间快,不良反应少。B组手术前插管和手术后拔管时,心率和血压明显升高,而术后拔管、清醒时间较前者慢,不良反应也较前者多。结论喉罩全麻用于腹腔镜胆囊手术既能达到气管插管全麻的效果,又操作简单,安全可靠,而且它的术后并发症也明显减少,术后睁眼时间、拔管时间也快,有助于维持患者的心血管功能稳定。与气管插管全麻比明显占优势,临床可以多应用。

Objective To explore the application value of laryngeal mask general anesthesia and endotracheal intubation general anesthesia in laparoscopic cholecystectomy.Methods We reviewed and analyzed 92 cases of laparoscopic cholecystectomy in our hospital from May 2016 to May 2017,were divided into two groups,46 cases in each group,laryngeal mask airway in group A and general anesthesia in group B.The heart rate and blood pressure of the two groups were carefully observed and compared,and the complications were also counted.Results The heart rate and blood pressure were not significantly changed in group A before and after the operation,and the postoperative extubation,awake time and adverse reaction were less.In group B,heart rate and blood pressure were significantly increased after intubation and extubation,while extubation and awake time were slower than those of the former.Conclusion The laryngeal mask general anesthesia for laparoscopic cholecystectomy can achieve the effect of general anesthesia with endotracheal intubation,simple operation,safe and reliable,and the postoperative complications are also significantly reduced,the time to open the eyes after operation is also fast,and help to maintain the stability of the cardiovascular function.Compared with tracheal intubation,general anesthesia is superior,clinical can be used more.
