评价彩色超声在疤痕妊娠诊治中的作用

Evaluation of the Role of Color Ultrasound in Diagnosis and Treatment of Scar Pregnancy

目的探讨评价彩色超声在疤痕妊娠诊治中的作用。方法选取2015年6月至2017年6月我院收治的116例瘢痕妊娠产妇临床资料进行回顾性分析,所有患者均进行彩色多普勒超声检查。结果本组116例子宫瘢痕妊娠患者经超声诊断结果显示,超声检测确诊患者101例,诊断准确率为87.07%;其中漏诊患者11例,漏诊率9.48%,误诊患者4例,误诊率3.45%。超声检查图像特点:典型孕囊型患者69例,占59.48%;混合性包块患者19例,占16.38%;孕囊样结构型28例,占24.14%。本组116例患者治疗后30d后,血流信号(β-HCG)明显降低,病灶显然缩小,而阻力指数显然升高与治疗前比较(P<0.05)。结论临床中采用彩色多普勒超声对子宫瘢痕妊娠患者进行检查,可为分娩方式选择、诊断治疗提供准确有效参考依据,提高产妇分娩安全性,值得临床借鉴。

Objective To evaluate the role of color ultrasound in the diagnosis and treatment of scar pregnancy. Methods From June 2015 to June 2017, 116 cases of maternal pregnancy were analyzed retrospectively. All patients underwent color Doppler ultrasonography. Results The diagnosis of 116 cases of uterine scar patients with ultrasound diagnosis showed that 101 cases were diagnosed by ultrasonography, the diagnostic accuracy rate was 87.07%; 11 cases were missed, the rate of missed diagnosis was 9.48%;4 cases were misdiagnosed, the misdiagnosis rate was 3.45% The Ultrasonographic features: 69 cases of typical pregnancy-type patients,accounting for 59.48%; 19 cases of mixed mass in patients, accounting for 16.38%; 28 cases of pregnancy-like structure, accounting for 24.14%. The blood flow signal(β-HCG) was significantly decreased after 30 days of treatment in 116 patients, and the lesion was significantly reduced and the resistance index was significantly higher than that before treatment(P<0.05). Conclusion Color Doppler ultrasonography is used to check the patients with uterine scar pregnancy. It can provide accurate and effective reference for the choice of delivery mode, diagnosis and treatment, and improve the safety of maternal delivery. It is worthy to be popularized.
