益脾清肺汤治疗咳嗽变异性哮喘的临床疗效评价及对炎性因子的调节作用

Clinical Evaluation of Yi PI Qingfei Decoction in Treating Cough Variant Asthma and Its Regulation on Inflammatory Factors

目的观察益脾清肺汤治疗咳嗽变异性哮喘(CVA)的临床疗效,并且观察其对炎性因子的调节作用。方法选取本院呼吸科门诊收治的CVA患者84例,将所选患者随机分为2组,对照组患者给予沙美特罗替卡松粉吸入剂联合孟鲁司特钠片治疗,观察组患者在对照组治疗的基础上采用益脾清肺汤治疗,2组均连续治疗3周。比较治疗前后2组患者咳嗽及哮喘症状评分、炎性因子指标、肺功能指标及临床疗效。结果观察组总有效率92.8%,对照组总有效率83.3%,两组比较有统计学意义(P<0.05);治疗后观察组IFN-γ较治疗前显著升高(P<0.05),IL-4、IL-8均较治疗前显著降低(P<0.05),并且较对照组变化幅度大(P<0.05)。治疗后观察组患者的肺功能指标FVC、FEV1、FEV1/FVC及PEF均较治疗前显著升高(P<0.05),而观察组的各项肺功能指标显著高于对照组(P<0.05);观察组治疗后咳嗽评分及哮喘评分均显著较治疗前高(P<0.05)。结论益脾清肺汤治疗CVA疗效显著,并且可以明显减轻机体气道炎症反应,抑制气道慢性非特异性炎症。

Objective To observe the clinical efficacy of Yi Pi Qingfei Decoction in the treatment of cough variantasthma(CVA), and to observe its regulatory effect on inflammatory factors. Methods 84 patients with CVA wererandomly divided into two groups. The patients in the control group were treated with salmeterol-fluticasone powderinhalation combined with montelukast sodium tablets. The patients in the observation group were treated with YiPi Qingfei Decoction on the basis of the treatment in the control group. Both groups were treated continuously for3 weeks. The symptoms of cough and asthma, inflammatory factors, pulmonary function and clinical efficacy werecompared between the two groups before and after treatment. Results The total effective rate of the observation groupwas 92.8%, and that of the control group was 83.3%. There was statistical significance between the two groups(P<0.05).After treatment, IFN-gamma in the observation group was significantly higher than that before treatment(P<0.05),and IL-4 and IL-8 were significantly lower than that before treatment(P<0.05), and the change was greater thanthat in the control group(P<0.05). After treatment, the lung function indexes FVC, FEV1, FEV1/FVC and PEF in theobservation group were significantly higher than those before treatment(P<0.05), while the lung function indexes inthe observation group were significantly higher than those in the control group(P<0.05);the cough score and asthmascore in the observation group were significantly higher than those before treatment(P<0.05). Conclusion Yi Pi QingfeiDecoction is effective in the treatment of CVA, and can significantly reduce airway inflammation and inhibit chronicnonspecific airway inflammation.
