探讨羊水栓塞致孕产妇死亡的病理诊断和羊水栓塞的预防措施

Exploring the Pathological Diagnosis of Maternal Mortality Caused by Amniotic Fluid Embolism and Preventive Measures for Amniotic Fluid Embolism

目的探讨羊水栓塞致孕产妇死亡的病理诊断和羊水栓塞的预防措施。方法随机选取本院2009年1月至2018年1月期间内接收的15例羊水栓塞患者作为研究对象,分析羊水栓塞的病理诊断方法,并提出临床预防措施。观察本研究所选患者及新生儿预后情况,并对比患者采取措施干预前后寒颤发生情况。结果 (1)本研究所选患者术后羊水栓塞的0例,产后出血的1例,死亡的6例,新生儿窒息的1例。羊水栓塞致死亡患者中,2例死于心肺功能衰竭,4例死于过敏性休克。(2)采取措施干预后患者总寒颤发生率低于干预前,比较P<0.05,差异具有统计学意义。结论羊水栓塞的临床病理诊断需要结合患者的临床特征以及实验室检查进行诊断,从而提高诊断的准确性,并准确的采取预防措施进行干预,以降低患者的死亡率,改善患者及新生儿的预后。

Objective To investigate the pathological diagnosis of maternal mortality caused by amniotic fluid embolism and preventive measures for amniotic fluid embolism.Methods Fifteen patients with amniotic fluid embolism received from January 2009 to January 2018 were randomly selected as the research object. The pathological diagnosis method of amniotic fluid embolism was analyzed and clinical preventive measures were proposed. Observing the prognosis of the selected patients and neonates in this study, and comparing the incidence of chills before and after intervention.Results Firstly,in the selected patients, 0 cases of postoperative amniocentesis, 1 case of postpartum hemorrhage, 6 cases of death, and 1 case of neonatal asphyxia. Of the patients who died of amniotic fluid embolism, 2 died of cardiopulmonary failure and 4 died of anaphylactic shock. Secondly,the incidence of total chills after intervention was lower than that before intervention, compared with P<0.05, the difference was statistically significant.Conclusion The clinical and pathological diagnosis of amniotic fluid embolism needs to be combined with the clinical features of patients and laboratory tests to improve the accuracy of diagnosis and preventive measures to reduce the mortality rate and improve the prognosis of patients and newborns.
