摘要
目的研究探讨经伤锥置钉短节段固定治疗胸腰段脊柱骨折的疗效。方法选取86例胸腰段脊柱骨折患者,按治疗方法不同平均分成对照组与观察组,对照组采用跨伤锥置钉固定术进行治疗,观察组采用经伤锥置钉短节段固定进行治疗,对比两组患者临床疗效。结果两组患者术前术后VAS评分、JOA评分均无统计学差异(P>0.05),两组患者术前Cobb角、伤锥椎体后缘和前缘高度比均无统计学差异(P> 0.05),术后与随访半年时两组患者Cobb角、伤锥椎体后缘高度比明显优于治疗前(P<0.05),且观察组均明显优于对照组(P<0.05)。结论经伤锥置钉短节段固定治疗胸腰段脊柱骨折疗效显著,不仅具有良好的复位、固定效果,并且术后远期能可更好的维持脊柱矫正度,具有较高临床推广应用价值。
Objective to study curative effect of short segment screw fixation with transtraumatic vertebra in treatment of thoracolumbar spine fracture. Methods choose 86 cases thoracolumbar spine fracture patients divide them into control group and observation group in average according to different treatment methods. Control group was treated with trans-traumatic vertebra nailing fixation, and observation group with trans-traumatic short segment screw fixation. Compare clinical effect of two groups. Results VAS and JOA scores showed no significant difference between two groups before and after operation(P> 0.05). Preoperative Cobb angle, height ratio of posterior and anterior edge of traumatic vertebra between two groups showed no significant difference,(P>0.05). Follow up half a year after operation showed, Cobb angle and posterior edge height ratio of traumatic vertebra between two groups was significantly better than before treatment(P<0.05), and that of observation group was significantly better than control group(P<0.05). Conclusion short segment screw fixation with transtraumatic vertebra has significant effect in treatment of thoracolumbar spine fracture, not only can achieve good reduction and fixation effect, but can maintain better spinal correction in long term after operation, which is of high clinical promotion and application value.
