自体角膜缘干细胞移植联合翼状胬肉切除的疗效及预后分析

Efficacy and Prognosis of Autologous Limbal Stem Cell Transplantation Combined with Pterygium Excision

目的分析自体角膜缘干细胞移植联合翼状胬肉切除的疗效及预后。方法在2016年2月到2018年3月开展本次研究,在研究期间总共选取161例200眼作为研究案例。根据结膜植片宽度进行分组,对于经线宽度为3 mm的患者划分为小植片组,对于经线宽度为5 mm的患者划分为大植片组。对比两组患者角膜刺激症状以及复发率。结果术后1 d与2 d时小植片组患者的角膜刺激症状明显轻于大植片组,数据差异突出,统计学结果有意义(P<0.05);术后3 d后两组患者角膜刺激症状无明显差异,统计学结果无意义(P>0.05);术后2个月两组患者术后复发率无明显差异,统计学结果无意义(P>0.05);术后3个月后小植片组患者的术后复发率明显高于大植片组,数据差异突出,统计学结果有意义(P<0.05)。结论自体角膜缘干细胞移植联合翼状胬肉切除的疗效突出,治疗中应当尽可能采取大经线宽度的结膜植片,从而达到降低术后刺激,提高患者生存质量,值得临床普及。

Objective To analyze the efficacy and prognosis of autologous limbal stem cell transplantation combined with pterygium excision. Methods The study was conducted from February 2016 to March 2018. A total of 161 cases of 200 eyes were selected as study cases during the study period. According to the width of the conjunctival graft, the patients with a warp width of 3 mm were divided into small pieces, and the patients with a warp width of 5 mm were divided into large groups. Corneal irritation symptoms and recurrence rates were compared between the two groups. Results The corneal irritation of the Xiaomiao group was significantly lower than that of the Dazhi tablet group at 1d and 2d after operation. The data were prominent and the statistical results were significant(P<0.05). The corneal irritation was not obvious after 3 days. The difference was statistically insignificant(P>0.05). There was no significant difference in postoperative recurrence rate between the two groups at the 2nd postoperative period. The statistical results were meaningless(P>0.05). After 3 months, the small graft group The postoperative recurrence rate of the patients was significantly higher than that of the large graft group. The data were prominent and the statistical results were significant(P<0.05). Conclusion The effect of autologous limbal stem cell transplantation combined with pterygium excision is outstanding. In the treatment, conjunctival grafts with large warp width should be adopted as much as possible, so as to reduce postoperative stimulation and improve the quality of life of patients, it is worthy of clinical popularization.
