第二产程分阶段自由体位分娩的临床探讨

Clinical Study on Staged Free Body Delivery in the Second Stage of Labor

目的探讨第二产程分阶段自由体位在产妇分娩中的应用价值。方法选取本院2017年11月至2018年8月接收的186例产妇,随机分为观察组与对照组,每组93例。指导对照组产妇采取膀胱截石位或常规仰卧位,指导观察组产妇采取分阶段自由体位。对比两组产妇分娩方式,并观察分娩基本情况。结果观察组自然分娩率较对照组更高,且阴道助产率、剖宫产率较对照组更低;观察组第二产程总时间、产妇自主用力时间、产后出血量较对照组均更低;比较P<0.05,有统计学意义。结论产妇分娩中第二产程采取分阶段自由体位可有效提高产妇阴道自然分娩几率,缩短第二产程时间,降低产后出血量,临床实用价值较高,值得加以推广。

Objective To explore the application value of the staged free body position in the second stage of labor in maternal delivery. Methods A total of 186 maternal women received from November 2017 to August 2018 were randomLy divided into observation group and control group, with 93 cases in each group. The control group was given a bladder lithotomy position or a conventional supine position to guide the observation group to adopt a staged free position. Compare the two groups of maternal delivery methods and observe the basic situation of childbirth. Results The natural delivery rate of the observation group was higher than that of the control group, and the vaginal assist rate and cesarean section rate were lower than those of the control group. The total time of the second stage of labor, the time of maternal self-administration, and the amount of postpartum hemorrhage in the observation group were more than those of the control group, P<0.05, with statistically significant. Conclusion The staged free body position in the second stage of labor delivery can effectively increase the maternal vaginal natural delivery rate, shorten the second stage of labor, reduce postpartum hemorrhage, and has higher clinical practical value, which is worthy of promotion.
