探讨同伴教育对鼻咽癌放疗患者张口锻炼依从性的影响

To Explore the Effect of Peer Education on Compliance of Nasopharyngeal Carcinoma Patients with Open Mouth Exercise after Radiotherapy

目的观察在鼻咽癌放疗患者中采取同伴教育对其张口锻炼的依从性分析。方法将我院耳鼻喉科近期(2017年10月至2018年10月)收治以鼻咽癌放疗为诊断的患者总计80例,随机数字法分成对照组(40例)与同伴组(40例),其中对照组患者采取常规健康教育干预,同伴组患者采取同伴教育干预。对照两组患者后张口锻炼依从性、张口困难程度,干预前、后及生存质量比较。结果同伴组与对照组患者张口锻炼依从性优率为(82.5%&42.5%),依从性良率(17.5%&47.5%),依从性差率(0%&10%),优良率(100%&90%),干预后Ⅰ级张口困难程度同伴组与对照组(75%&27.5%),Ⅱ级张口困难程度(25%&47.5%),Ⅲ级张口困难程度(0%&15%),Ⅳ级张口困难程度(0%&10%),干预后,同伴组QLQ-CCC评分明显高于对照组,差异具有统计学意义(P<0.001)。结论在鼻咽癌放疗患者中,采取同伴教育,可明显提高患者对张口锻炼的依从性,减轻张口困难的程度,提高生存质量,效果理想。

Objective To observe the compliance of peer education in nasopharyngeal carcinoma patients undergoing radiotherapy with mouth opening exercise.Methods A total of 80 patients with nasopharyngeal carcinoma diagnosed by radiotherapy were admitted to otolaryngology department of our hospital in recent years(October 2017-October 2018).They were randomly divided into control group(40 cases)and peer group(40 cases).The patients in control group were given routine health education intervention,while those in peer group were given peer education intervention.Comparisons were made between the two groups in compliance with post-oral opening exercise,difficulty in opening mouth,and quality of life before and after intervention.Results The excellent rate of compliance of peer group and control group was 82.5%-42.5%.The good rate of compliance was 17.5%-47.5%.The poor rate of compliance was 0%-10%.The good rate was 100%-90%.After intervention,the peer group and control group had difficulty in opening mouth at grade I(75%-27.5%),the second group had difficulty in opening mouth at grade II(25%-47.5%),the third group had difficulty in opening mouth at grade III(0%-15%)and the fourth group had difficulty in opening mouth.After intervention,the QLQ-CCC score of the peer group was significantly lower than that of the control group(P<0.001).Conclusion Peer education can significantly improve the compliance of patients with nasopharyngeal carcinoma to mouth opening exercise,reduce the difficulty of mouth opening,and improve the quality of life.
