摘要
目的探讨分析经皮肾镜和输尿管软镜取石术针对输尿管上段结石伴肾积水的临床疗效。方法回顾2015年1月至2017年12月间我院收治的92例输尿管结石伴肾积水患者临床资料,并将采用经皮肾镜碎石取石术(PCNL)治疗的46例设为对照组,将采用输尿管软镜碎石取石术(FURL)治疗的46例设为观察组,观察对比两组临床疗效。结果观察组手术时间、术后血尿持续时间明显短于对照组,术后第3天CRP水平明显低于对照组,术后并发症发生率明显低于对照组,且差异均具有统计学意义(P<0.05);两组患者手术成功率差异无统计学意义(P>0.05)。结论输尿管软镜下碎石取石术治疗输尿管上段结石伴肾积水的临床疗效显著,并且具有操作方便、损伤小等优点,具有临床推广应用价值。
Objective To explore the clinical effects of percutaneous nephrolithotomy and flexible ureteroscope lithotrity in the treatment of upper ureteral calculi complicated with hydronephrosis.Methods Clinical data of 92 cases of ureteral calculi complicated with hydronephrosis who were treated in Nuclear Industry 416 Hospital from January2015 to December 2017 were reviewed in this study.The cases were evenly divided into two groups:a control group(n=46)in which percutaneous nephrolithotomy(PCNL)was adopted and an observation group(n=46)in which flexible ureteroscope lithotrity(FURL)was adopted.The clinical effects were compared between the two groups.Results The operation duration and postoperative hematuria duration of the observation group were significantly shorter than those of the control group.The CRP level 3 days after operation in the observation group was significantly lower than that in the control group.The postoperative complication rate in the observation group was significantly lower than that in the control group.The differences above were statistically significant(P<0.05).There was no significant difference in the success rate between the two groups(P>0.05).Conclusion The application of FURL in the treatment of patients with upper ureteral calculi complicated with hydronephrosis has significant clinical effects and boasts of convenient operation and small damage.Therefore,it is worthy of widespread application in clinical practice.
