骶髂关节CT检查诊断早期强直性脊柱炎临床效果观察 被引量:2

摘要 目的对在早期强直性脊柱炎的临床诊断中骶髂关节CT检查的应用效果进行分析,以期为进一步提高临床诊断准确率、为患者争取最佳治疗时机提供参考。方法将我院骨科于2016年6月至2018年6月期间接收的68例临床疑有早期强直性脊柱炎但尚未确诊患者纳入本次研究观察范围,先后对所有患者行骶髂关节X线平片、CT扫描检查,将CT图像与临床资料及X线平片检查结果进行对比观察。结果 CT扫描检查病情诊断准确率、分级检出率中Ⅱ级和Ⅲ级检出率均显著高于X线平片检查(P<0.05)。结论 X线平片及CT在对早期强直性脊柱炎的诊断中均具有一定临床诊断价值,但在炎症分级方面,CT检查检出率更高,对于临床高度疑诊而平片显示正常或可疑者的鉴别及确诊具有极其重要的意义。 Objective To Analyze the effects of the application of CT examination of the sacroiliac joint in clinical diagnosis of early ankylosing spondylitis,aiming to provide references for further improvement of clinical diagnosis accuracy and determination of the best treatment opportunity for the patients.Methods 68 suspect patients with ankylosing spondylitis who were treated in North Asian Orthopedics Hospital of Beijing from June 2016 to June2018 were included for study.All of them successively received X-ray examination and CT scan of sacroiliac joints.The CT images,clinical data,and examination results of X-ray were compared and analyzed.Results Compared with X-ray examination,CT scan achieved significantly higher diagnosis accuracy as well as significantly higher detection rates of ClassⅡand ClassⅢof ankylosing spondylitis(P<0.05).Conclusion Both X-ray examination and CT scan have certain clinical value in the diagnosis of early ankylosing spondylitis.However,CT examination enjoys higher detection rates in terms of inflammatory grading.Therefore,it is extremely significant for the people who are highly suspect patients with ankylosing spondylitis according to clinical diagnosis but normal ones or suspect patients according to X-ray results.
