支气管肺泡灌洗液中NSE检测于小细胞肺癌诊治中的价值

摘要 目的探讨支气管肺泡灌洗液神经元型特异性烯醇化酶(NSE)测定对小细胞肺癌诊断及治疗中的临床价值。方法纳入我院自2016年1月至2018年1月经气管镜检查确诊小细胞肺癌(SCLC)患者83例为实验组,随机选取同期经气管镜检查确诊非小细胞肺癌(NSCLC)患者83例为对照组,收集两组血清及支气管灌洗液中NSE数据值,对组间血清及支气管肺泡灌洗液中NSE含量及实验组内两种标本NSE值进行对比分析。从83例小细胞肺癌患者中选取接受化疗2周期并经影像学检查确认有效的患者67例,将治疗后患者血清及支气管灌洗液NSE值与治疗前行对比分析。结果实验组血清及支气管灌洗液中NSE含量均明显高于对照组(P<0.05),实验组内支气管灌洗液中NSE含量明显高于血清NSE含量(P<0.05)。经治疗后血清及支气管灌洗液中NSE含量均明显降低(P<0.05),且支气管灌洗液中NSE含量降低程度大于血清NSE含量降低程度(P<0.05)。结论支气管灌洗液NSE检测相对于血清NSE检测对小细胞肺癌的诊断及疗效评价更有价值。 Objective To investigate the clinical value of NSE(neuron-specific enolase)assay in the diagnosis and treatment of small cell lung cancer(SCLC).Methods Included in our hospital from 2016.1 to 2018.1 menstrual bronchoscope examination confirmed 83 cases of small cell lung cancer patients as experimental group,in the same period were randomly selected by bronchoscope examination diagnosis of non-small cell lung cancer patients 83 cases as control group,and collect two groups of NSE in serum and bronchial lavage fluid data values,between the group and NSE content in bronchoalveolar lavage fluid and serum in the experimental group two specimens of NSE values were analyzed.Sixty-seven patients with small-cell lung cancer who received 2 cycles of chemotherapy and were confirmed effective by imaging examination were selected from 83 patients with small-cell lung cancer.Serum and bronchial lavage NSE values of the patients after treatment were compared with those before treatment.Results The content of NSE in the serum and broncholavage fluid in the experimental group was significantly higher than that in the control group(P<0.05),and the content of NSE in the broncholavage fluid in the experimental group was significantly higher than that in the serum(P<0.05).After treatment,the NSE content in serum and broncholavage solution was significantly reduced(P<0.05),and the degree of NSE content in broncholavage solution was greater than that in serum NSE content(P<0.05).Conclusion Compared with serum NSE,broncholavage fluid NSE is more valuable in diagnosis and efficacy evaluation of small cell lung cancer.
