加温湿化高流量鼻导管通气对急性低氧性呼吸衰竭患者的疗效及护理观察 被引量:3

Therapeutic Effect and Nursing Observation of Warming and Humidifying High Flow Nasal Catheter Ventilation in Patients with Acute Hypoxic Respiratory Failure

摘要 目的分析与研究加温湿化高流量鼻导管通气对急性低氧性呼吸衰竭患者的护理以及治疗效果。方法选取我院呼吸内科接收诊治的100例急性低氧性呼吸衰竭患者,利用随机数字表法将其平均分为对照组和观察组,各50例。对照组患者采用低流量氧疗,每日维持16 h,吸氧浓度保持为25%~30%;对观察组患者应用鼻导管氧疗,吸氧浓度保持在30%左右,流量为3~8 L/min。分析对比两组患者的转机械通气率、吸氧相关不良事件发生率以及护理难易程度评分。结果对照组的转机械通气率为62.00%(31/50),观察组为14.00%(7/50),观察组明显低于对照组,差异具有统计学意义(P<0.05);对照组和观察组的吸氧相关不良事件发生率分别为72.00%(36/50)和24.00%(12/50),组间差异具有统计学意义(P<0.05);观察组的护理难易程度评分为(8.78±1.61)分显著高于对照组的(5.99±1.52)分,差异具有统计学意义(P<0.05)。结论对急性低氧性呼吸衰竭患者应用加温湿化高流量鼻导管通气进行治疗,可较快纠正患者低氧血症,显著降低相关不良事件发生率和机械通气的风险,大幅提高患者的依从性和舒适程度,从而减轻相关护理工作的负担,值得广泛推广并应用。 Objective To analyze and study the nursing and therapeutic effect of hyperthermia humidification and high flow nasal catheter ventilation in patients with acute hypoxic respiratory failure.Methods One hundred patients with acute hypoxic respiratory failure received and treated in Respiratory Medicine Department of our hospital were divided into control group and observation group with 50 cases in each group by random number table method.The patients in the control group were treated with low flow oxygen therapy,which lasted 16 hours a day,and the concentration of oxygen inhalation was 25%~30%.The patients in the observation group were treated with nasal catheter oxygen therapy,and the concentration of oxygen inhalation was about 30%.The flow rate was 3~8 L/min.The rate of mechanical ventilation,incidence of adverse events related to oxygen inhalation and nursing difficulty score were analyzed and compared between the two groups.Results The conversion rate of mechanical ventilation was 62.00%(31/50)in the control group and 14.00%(7/50)in the observation group,which was significantly lower than that in the control group(P<0.05).The incidence of adverse events related to oxygen uptake in the control group and the observation group were 72.00%(36/50)and 24.00%(12/50),respectively.There was a significant difference between the two groups(P<0.05).The score of nursing difficulty in the observation group was(8.78±1.61)significantly higher than that in the control group(5.99±1.52),and the difference was statistically significant(P<0.05).Conclusion The application of warming and humidifying high flow nasal catheter ventilation in the treatment of acute hypoxic respiratory failure can quickly correct hypoxemia,significantly reduce the incidence of related adverse events and the risk of mechanical ventilation,significantly improve the compliance and comfort of patients,thereby reducing the burden of related nursing work.It should be widely popularized and applied.
