喷砂处理在重度老年慢性牙周炎治疗中的辅助作用

目的:探讨喷砂处理在重度老年慢性牙周炎治疗中的辅助作用。方法:选择2018年1月~12月在我院口腔科接受治疗的30例重度老年慢性牙周炎患者作为研究对象,所有患牙都经过规范的超声龈上洁治和超声龈下刮治,1月后复查,仍有深牙周袋者,分为实验组和对照组,实验组患牙进行根面平整联合喷砂处理治疗,对照组仅进行根面平整治疗。比较两组患牙在治疗前、治疗后3个月、6个月的临床牙周指标(菌斑指数PLI、出血指数BI、探诊深度PD、附着丧失AL),记录根分叉位点(上颌磨牙颊侧正中位点、下颌磨牙颊舌侧正中位点、上颌磨牙腭侧近远中位点)治疗后与治疗前PD的差值。同时记录在治疗过程中及治疗后1周的患牙疼痛程度。结果:治疗后两组患牙的临床指标都较治疗前有明显改善,实验组效果优于对照组,两组之间的对比有统计学意义(P<0. 05)。3个月、6个月复诊时,实验组根分叉位点的PD差值均大于对照组,差异有统计学意义(P<0. 05)。实验组疼痛评分明显低于对照组(P<0. 05)。结论:根面平整联合喷砂处理治疗可提高老年慢性牙周炎的治疗效果,改善老年患者牙周临床指标,并且可降低患者在治疗过程中的不适感,在老年慢性牙周炎的治疗中有较广泛的应用前景。

Objective:To investigate the auxiliary.effect of blasting in the treatment of severe chronic periodontitis in the elderly.Methods:30 cases of severe elderly patients with chronic periodontitis treated in the Department of Stomatology in our hospital from January to December 2018 were selected as subjects.All the teeth were subjected to standardized supragingival and subgingival scaling.After a month assessment,patients with deep periodontal pockets were divided into.experimental group and control group.The experimental group were subjected to root planning combined with blasting treatment,and the control group were only subjected to root planning.The clinical periodontal indexes(plaque index PLI,bleeding index BI,probing depth PD,attachment loss AL)of the two groups of teeth were recorded and compared before treatment,3 months and 6 months after treatment.The PD difference of the furcation site(the median site of the buccal side of the maxillary molar,the median site of the buccal lingual side of the mandibular molar,and the proximal and distal site of the maxillary molar)were recorded.The degree of pain in teeth during and 1 week after treatment was also recorded by VAS.Results:After treatment,the periodontal indexes both groups were significantly improved compared with those before treatment,The effect of the experimental group was better than that of the control group.At 3 months and 6 months follow-up visit,the comparison between the two groups was statistically significant(P<0.05).At 3 months,6 months of follow-up,the PD difference in the furcation site of the experimental group was greater than that of the control group(P<0.05).The pain score of the experimental group was significantly lower than that of the control group(P<0.05).Conclusion:Root planning combined with subgingival blasting can improve the therapeutic effect of chronic periodontitis in the elderly,improve the periodontal.It has a wide application prospect in the treatment of elderly chronic periodontitis.
