清洁开颅术后医院感染影响因素研究

Influencing factors for nosocomial infection in patients undergoing clean craniotomy

目的 探究神经外科清洁开颅术后引起患者医院感染的影响因素,为降低医院感染的发生提供行之有效的预防控制措施。方法 回顾性分析医院2014-2016年神经外科全部出院患者的临床资料,分为术后感染组与未感染组,单因素及多因素logistic回归分析清洁开颅术后患者医院感染的影响因素。结果 2014-2016年神经外科行清洁开颅手术患者1 146例,发生医院感染323例,医院感染率为28.18%。三年医院感染率比较,差异无统计学意义(P=0.056);多因素logistic回归分析显示心血管疾病、留置鼻胃管、2次以上入住ICU、留置导尿管时间、留置脑室引流、手术时间和住院时间是医院感染的独立影响因素。结论 清洁开颅术后患者的医院感染率较高,通过控制医院感染独立影响因素,采取行之有效地控制措施,可降低医院感染的发生。

OBJECTIVE To explore the influencing factors for nosocomial infection in neurosurgery department patients undergoing the clean craniotomy so as to put forward effective prevention measures and reduce the incidence of nosocomial infection.METHODS The clinical data of the patients who were discharged from the neurosurgery department in 2014-2016were retrospectively analyzed,and the patients were divided into the infection group and the non-infection group.The univariate analysis and multivariate logistic regression analysis were performed for the influencing factors for the postoperative nosocomial infection in the patients undergoing the clean craniotomy. RESULTS Of 1146patients who underwent the clean craniotomy in the neurosurgery department from 2014to 2016,323had nosocomial infection,with the infection rate 28.18%.There was no significant difference in the infection rate among the three years(P=0.056).The multivariate logistic regression analysis showed that the independent influencing factors for the nosocomial infection included cardiovascular diseases,nasogastric tube indwelling, ICU stay no less than 2times,urinary catheter indwelling time,ventricular drainage indwelling,operation duration and length of hospital stay.CONCLUSIONThe incidence of nosocomial infection is high in the patients undergoing the clean craniotomy.It is necessary to control the independent influencing factors for the nosocomial infection and take effective prevention measures so as to reduce the incidence of nosocomial infection.
