摘要
目的研究某院就诊患儿TORCH感染的状况。方法选取2016年1月-2017年1月在某院儿科门诊和住院的230例患儿为研究对象,采用化学发光免疫测定仪对患儿血清TORCH-IgM进行检测。分析230例患儿TORCH感染的感染模式及总感染率。结果 230例患儿TORCH检测结果,巨细胞病毒-免疫球蛋白M(CMV-IgM)、单纯疱疹Ⅰ/Ⅱ型-IgM(HSV-IgM)、风疹病毒-IgM(RV-IgM)、弓形虫-IgM(TOX-IgM),检测阳性率分别为:16.96%、9.13%、1.74%、0.87%,其中CMV-IgM的阳性率最高,TOX-IgM最低;CMV-IgG、HSV-IgG、RV-IgG、TOX-IgG的阳性率分别为:90.00%、83.91%、59.13%、0.87%。合并感染占6.96%(16/230),分别为CMV+HSV、CMV+RV、HSV+RV、CMV+HSV+RV、CMV+HSV+TOX;不同年龄段患儿TORCH-IgM感染率比较差异有统计学意义(χ2=5.024,P=0.025);在1~28d年龄段确诊为先天性的7例TORCH-IgM感染婴幼儿中,巨细胞病毒性肺炎4例,早产儿、肝炎、听力不健全各1例。其他年龄段的患儿TORCH-IGM感染状况中,脑炎3例、黄疸21例、巨细胞病毒肝炎7例、溶血性贫血5例、血小板减少性紫癜7例、疱疹性角膜结膜炎1例、肝脾肿大3例。其中CMV感染的32例,HSV感染的18例,RV感染的5例,TOX感染的1例。结论住院患儿TORCH感染较为常见,尤其是其中的CMV感染率较高。
OBJECTIVE To investigate the prevalence of TORCH infection in hospitalized children of a hospital.METHODS A total of 230 children who were treated and hospitalized in pediatrics department from Jan 2016 to Jan2017 were recruited as the study objects,the serum TORCH-IgM was detected for the children by using chemiluminescence immunoassay analyzer,and the modes of TORCH infection and total infection rate of the 230 children were observed.RESULTS The result of TORCH test of the 230 children showed that the positive rates of cytomegalovirus-immunoglobin M(CMV-IgM),type herpes simplexⅠ/Ⅱ-IgM(HSV-IgM),rubella virus-IgM(RVIgM)and Toxoplasma gondii-IgM(TOX-IgM)were 16.96%,9.13%,1.74%and 0.87%,respectively;the positive rate of CMV-IgM was the highest,TOX-IgM the lowest;the positive rates of CMV-IgG,HSV-IgG,RV-IgG and TOX-IgG were 90.00%,83.91%,59.13%and 0.87%.The children who had mixed infection accounted for6.96%(16/230),including CMV+HSV infection,CMV+RV infection,HSV+RV infection,CMV+HSV+RV infection and CMV+HSV+TOX infection.There was significant difference in the infection rate of TORCH-IgM among the different age groups of children(χ~2=5.024,P=0.025).Among the 7 children who were aged between 1 and 18 days and diagnosed with geneogenous TORCH-IgM infection,there were 4 cases of CMV pneumonia,1 case of premature,1 case of hepatitis and 1 case of hearing impairment.Among the children with TORCH-IgM infection in other age groups,there were 3 cases of encephalitis,21 cases of jaundice,7 cases of CMV hepatitis,5 cases of hemolytic anemia,7 cases of thrombocytopenic purpura,1 case of herpetic keratoconjunctivitis and 3 cases of hepatosplenomegaly.There were 32 cases of CMV infection,18 cases of HSV infection,5 cases of RV infection and 1 case of TOX infection.CONCLUSION The TORCH infection is prevalent among the hospitalized children,and the incidence of CMV infection is relatively high.
