脑出血后颅内感染患者血液流变学及炎症因子变化研究

Hemorheological indexes and inflammatory factors of intracranial hemorrhage patients with intracranial infection

目的分析脑出血后颅内感染患者血液流变学指标及炎症因子水平变化,并探究患者预后。方法选择2016年5月-2018年4月山西省人民医院收治的脑出血后发生颅内感染患者56例为研究组,并选择同期脑出血后未发生颅内感染患者74例为对照组,检测两组患者血液流变学指标(全血黏度高切、全血黏度中切、全血黏度低切、血浆黏度、红细胞聚集指数)及炎症因子C-反应蛋白(CRP)和降钙素原(PCT)水平,并比较研究组内不同预后患者指标的差异。结果研究组患者血液流变学指标水平高于对照组(P<0.05),炎症因子指标水平高于对照组(P<0.05);研究组不同预后患者血液流变学及炎症因子指标水平比较差异有统计学意义(P<0.05),其中治愈患者指标水平低于好转、无效或加重患者(P<0.05)。结论全血黏度高切、全血黏度中切、全血黏度低切、血浆黏度、红细胞聚集指数等血液流变学指标及CRP、PCT水平与脑出血后颅内感染的发生及患者预后关系密切,以上指标检测对脑出血后颅内感染具有一定的诊断价值。

OBJECTIVE To investigate the changes of hemorheological indexes and inflammatory factors of the intracranial hemorrhage patients with intracranial infection and explore the prognosis of the patients.METHODS Totally 56 intracranial hemorrhage patients who were complicated with intracranial infection and treated in Shanxi People′s Hospital from May 2016 to Apr 2018 were chosen as the study group,meanwhile,74 intracranial hemorrhage patients who were not complicated with intracranial infection were set as the control group.The hemorheological indexes(high blood viscosity,whole blood viscosity,low blood viscosity,plasma viscosity,erythrocyte aggregation index)and inflammatory factors C-reactive protein(CRP)and procalcitonin(PCT)of the two groups of patients were detected,and the above indexes were compared among the patients with different outcomes in the study group.RESULTS The levels of hemorheological indexes of the study group were significantly higher than those of the control group(P<0.05);the levels of inflammatory factors of the study group were significantly higher than those of the control group(P<0.05).There were significant differences in the hemorheological indexes and levels of inflammatory factors among the patients with different outcomes in the study group(P<0.05);the levels of above indexes of the cured patients were significantly lower than those of the improved,ineffective and aggravated patients(P<0.05).CONCLUSION The hemorheological indexes such as high blood viscosity,whole blood viscosity,low blood viscosity,plasma viscosity and erythrocyte aggregation index,CRP and PCT are closely associated with the intracranial infection and prognosis of the intracranial hemorrhage patients,and the above indexes have certain value in diagnosis of the intracranial infection in the intracranial hemorrhage patients.
