可吸收螺钉在髁突矢状骨折中的应用 被引量:4

摘要 目的:检验可吸收螺钉作为内固定材料手术治疗下颌髁突矢状骨折(sagittal fractures of the mandibular condyle,SFMC)的效果。方法:实验组5例单侧SFMC患者采用耳屏前切口,牵引螺钉辅助复位,可吸收螺钉侧向固定骨折,术前、术中、术后进行临床和放射学检查、评价,平均追踪随访12个月;对照组为前期病历资料记载完整、病情类似、采用钛螺钉侧向固定治疗的5例SMFC患者。比较两组患者术后3个月时最大开口度、向患侧和健侧侧向运动度。结果:所有患者手术过程和术后的愈合顺利,咬合关系恢复良好,两组中各有1例发生暂时性面瘫,术后3个月时,下颌的运动度基本恢复正常。实验组与对照组相比,最大开口度、向患侧和健侧侧向运动度差异均无统计学意义。术后即刻CT检查提示,两组患者术后髁突骨折块均复位固定良好,没有发生骨折再次移位;术后6个月时,两组患者髁突形态基本恢复正常,骨折线消失,对照组高密度内固定材料无变化,实验组低密度内固定材料部分被骨质取代。结论:可吸收螺钉作为SFMC手术治疗的内固定材料,可以达到与钛螺钉相似的治疗效果。但可吸收螺钉可以无害化降解、吸收,避免了二次手术取出螺钉引起的创伤。
