CT引导微波凝固疗法在肺肿瘤方面的应用 被引量:4

摘要 目的 探讨CT引导下经皮经肺微波凝固治疗肺肿瘤的方法及应用价值。方法 CT引导机型SIEMENSSO MATOMACR型CT机 ;微波治疗仪一台 ,以 2 45 0MHz的微波 6 5W辐射 6 0s。对三头同种同体重猪行经皮肺穿实验研究 ,观察CT表现和病理变化。对 2 0例肺肿瘤患者 2 8个病灶CT引导下经皮经肺微波凝固治疗 ,行CT平扫和增强扫描 ,观察肿瘤大小及坏死情况。结果 动物实验猪肺微波凝固范围约 30 .1mm× 2 0 .1mm。对肺肿瘤患者治疗后随访 ,结果 2 8个病灶治疗后 4个病灶增大 ;8个病灶轻度缩小 <1/ 4,13个病灶明显缩小 <1/ 2 ,3个病灶大部消失 ,有效率 (CR +PR )为 5 7.1%。无明显的并发症。结论 CT引导下经皮微波凝固疗法治疗周围型肺肿瘤是一种定位准确、高效、微创、安全的新方法。 Objective To evaluate the methods and clinical value of percutaneous microwave coagulation therapy for lung tumor by CT-guided. Methods CT-guided percutaneous puncture and 2450MHz microwave equipment were utilized. 65W 60 second irradiation was used to treat peripheral type lung tumor, after we studied three same weight and kinds of pigs. Results In pigs experiment the microwave coagulation lesion showed 30.1mm×20.1mm. In patients group by 3-24 months follow-up, 4 nodules became larger, 24 nodules decreased 1/4 diminution in size including 12 cases, 13 cases 1/2 diminution and 3 cases almost disappeared. CT contrast scaning and biopsy showed tumor necrosis. CR+PR was 57.1%. No serious complications occurred. Conclusion Percutaneous microwave coagulation therapy by CT-guided was an effective, mini-trauma and safe treatment for lung tumor.[
