摘要
目的 :探讨HPV6/ 11、16/ 18在外阴癌组织中的感染情况及与p5 3、MDM2蛋白表达的关系。方法 :用原位杂交法 (ISH)检测HPV6/ 11、16/ 18在 30例外阴癌、2 1例外阴上皮内瘤变 (VIN)及 10例外阴正常皮肤组织中的表达。同时用免疫组化SP法检测p5 3、MDM2蛋白的表达。结果 :HPV6/ 11、16/ 18在外阴癌、VIN中的阳性表达率分别为 60 % (18/30 )、33.33% (10 / 30 ) ,4 2 .86% (9/ 2 1)、2 8.5 7% (6/ 2 1) ,正常对照组没有表达。p5 3、MDM2蛋白在外阴癌、VIN、正常对照组中的阳性表达率分别为 63.33% (19/ 30 )、4 0 .0 0 % (12 / 30 ) ,4 7.62 %(10 / 2 1)、5 2 .38% (11/ 2 1) ,0 % (0 / 10 )、0 % (0 / 10 )。HPV6/ 11的表达在外阴癌组、VIN组与正常组差异有显著性 (P <0 .0 5 ) ,外阴癌组HPV16/ 18表达与正常组差异有显著性 (P <0 .0 5 )。外阴病变各组p5 3、MDM2与正常组差异有显著性 (P <0 .0 5 )。结论 :HPV6/ 11、HPV16/ 18、p5 3、MDM2蛋白在外阴组织的不同病变中表达均差异有显著性 ,在外阴癌的发生发展中HPV感染、p5
Objective:To investigate the infection of HPV6/11,HPV16/18 and the expression of p53,MDM2 in the development of vulvar carcinoma.Methods:The expressions of HPV6/11 and HPV16/18 DNA in 30 cases of vulvar carcinoma,in 21 cases of vulvar intraepithelial neoplasia (VIN) and in 10 cases of normal skin epithelium were detected using in site hybridization method.p53,MDM2 were stained immunohistochemically in above three groups.Results:The positive rates of HPV6/11 and HPV16/18 in vulvar carcinoma and VIN were 60%(18/30),33.33%(10/30);42.86%(9/21),28.57%(6/21) respectively,and there was no expression of HPV6/11 or HPV16/18 in normal skin epithelium.There were significant differences between the normal skin epithelium and vulvar lesions (P<0.05).The expression rates of p53,MDM2 in above three groups were 63.33%(19/30),40.00%(12/30);47.62%(10/21),52.38%(11/21);0%(0/10),0%(0/10)respectively,with significant difference among the vulvar lesions and normal skin epithelium (P<0.05).No significant difference can be established among p53,MDM2 in positive and negative HPV6/11 and 16/18.There was no significant difference between the expression of p53,MDM2.Conclusions:The expressions of HPV6/11,HPV16/18,p53 and MDM2 are significantly different among different vulvar lesions.The infection of HPV,mutation of p53 and the overexpression of MDM2 may play a role in the progress of vulvar carcinoma.
