摘要
目的:探讨抑癌基因PTEN(phosphatase and tensin homolog deleted in chromosome10),P^(21waf1)在鼻咽癌发病中的作用及其与EB病毒的关系。方法:分别应用免疫组化(SP)染色法检测鼻咽癌组,慢性鼻咽炎组,鼻咽部不典型增生或癌前状态组中PTEN、P^(21waf1)的表达及分布情况。结果:慢性鼻咽炎组PTEN蛋白阳性表达率(20/25,80.0％)高于不典型增生或癌前状态组(8/11,72.7％)及鼻咽癌组(32/78,41.0％)。Ⅰ～Ⅱ期鼻咽癌PTEN蛋白阳性表达高于Ⅲ～Ⅳ期。慢性鼻咽炎组P^(21waf1)蛋白阳性表达率(6/25,24.0％)低于不典型增生或癌前状态组(4/11,36.4％)和鼻咽癌组(60/78,76.9％)。Ⅰ～Ⅱ期鼻咽癌P^(21waf1)蛋白阳性表达率低于Ⅲ～Ⅳ期,发生淋巴结转移鼻咽癌组P^(21waf1)阳性表达率高于无淋巴结转移组的阳性表达率。结论:PTEN的失活或降低与鼻咽癌发生及临床分期有关,是鼻咽癌发生发展的早期事件。P^(21waf1)蛋白表达增多与鼻咽癌发生及发展进程、临床分期、淋巴结转移有关,可能是鼻咽癌发生、发展过程中的晚期事件。
Objective: To investigate the expression and distribution of PTEN (phosphatase and tensin homolog deleted in chromosome ten) and P21waf1 proteins in nasopharyngeal arcinoma. Methods: The expressions of PTEN and P21waf1 were determined by immunoenzyme technic in 78 patients with nasopharyngeal carcinoma (NPC), 25 cases with chronic nasopharyngitis (CN) and 11 cases with pre-cancerous lesions (PI). Among 78 cases (60 males and 18 females, ranging in age from 10 to 70 years old) with nasopharyngeal carcinoma, 25 were Ⅰ -Ⅱ stage, 53 Ⅲ -Ⅳ stage. Results: The positive rate of expression of PTEN protein was 41. 0% (32/78) in NPC group, 80. 0% (20/25) in CN group and 72. 7% (8/11) in PI group respectively. The positive rate of expression of P21waf1 protein was 76. 9% (60/78) in NPC group, 24. 0%(6/25) in CN group and 36. 4% (4/11) in PI group respectively. The expression of PTEN in stage of Ⅰ -Ⅱ (64. 0%) higher than that in stage Ⅲ -Ⅳ (30. 2%). The positive rate of P21waf1 was 60. 0% in stage of Ⅰ -Ⅱ and 84. 9% in stage of Ⅲ -Ⅳ. Conclusion: The losing of PTEN protein and increase of P21waf1 protein should be important roles in the etiology of NPC.
