变应性鼻炎白细胞祖代细胞和相关细胞因子的检测及意义 被引量:14

摘要 目的:探讨骨髓和外周血中的嗜酸粒细胞祖代细胞和成熟嗜酸粒细胞反应在变应性鼻炎发病中的作用和机制。材料和方法:20例常年性变应性鼻炎和10例对照外周血标本,以ELISA检测血清中的IL-5,以流式细胞仪检测外周血中的白细胞祖代细胞(CD_(34)、CD_(45)双阳性)。动物实验采用6~8周雄性SD大鼠,随机分成实验组10只和对照组10只,以卵清蛋白致敏激发制成变应性鼻炎模型,记数外周血和骨髓中白细胞比例,检测外周血和骨髓中的IL-5。结果:患者外周血中白细胞祖代细胞和IL-5均显著高于对照组(P<0.05)。大鼠变应性鼻炎模型骨髓中嗜酸粒细胞、嗜碱粒细胞、外周血中嗜酸粒细胞显著高于对照组(P<0.05);变应性鼻炎模型骨髓和外周血中IL-5显著高于对照组(P<0.05)。结论:骨髓中嗜酸粒细胞的增殖分化和向外周血及呼吸道粘膜的释放和迁移,可能在呼吸道炎症发病中发挥重要的作用。 Objective: To evaluate the roles of progenitors of leucocytes and mature eosinophils in the pathogenesis of allergic rhinitis. Method: Twenty pateints of allergic perennial rhinitis (AR) and ten control subjects were evaluated. The progenitor cells (CD34 and CD45) in peripheral blood were detected by flow cytometry, and the levels of IL-5 in peripheral blood were determined by ELISA respectively. Twenty SD rats were divided into allergic rhinitis group and control group randomly. The leucocytes in the smear of peripheral blood and bone marrow were counted, and the levels of IL-5 were detected by ELISA. Result: The ratio of progenitors to white blood cells of peripheral blood and IL-5 were significantly higher in AR patients (0. 95±0. 38%) than that in control group (0. 12±0. 05%) (P<0. 05). The ratio of eosinophils to white blood cells bone marrow was significantly higher in AR rats (53. 35±10. 10%) than that in cotrol group (34. 50±8. 08%) (P<0. 05). The levels of IL-5 bone marrow were significantly higher in rats (9. 67±1. 21 pg/ml) than that in control rats (7. 76±1. 05pg/ml)(P<0. 05). The ratio of eosinophils to white blood cells in peripheral blood was significantly higher in AR rats (4. 12±1. 05%) than that in control rats (1. 52±0. 56%) (P<0. 05). The levels of IL-5 in peripheral blood was significantly higher in AR rats (8. 05±1. 60pg/ml) than that in control rats (4. 56±1. 30pg/ml) (P<0. 05). Conclusion: The bone marrow repsonse of eosinophils, migrating into peripheral blood, and infiltrating into nasal mucosa should be play important role in the pathogenesis of allergic rhinitis.
