第一跖底动脉的外科解剖

Surgical Anatomy of the First Plantar Metatarsal Artery

目前第二趾移植和(?)趾甲皮瓣移植重建手功能的术式,常采用足背动脉与第一跖背动脉为血管蒂。但该血管蒂常有变异,据临床的实践经验,改用第一跖底动脉是一种可行的方法。但分离第一跖底动脉难度较大,故在55例足标本和13例血管铸型标本上观察了第一跖底动脉有关的外科解剖,讨论了利用第一跖底动脉的应用解剖学要点。

During operations of the second toe transfer and the great toe nail flap graft-ing,the first plantar metatarsal artery is usually employed in case of abnormalfirst dorsal metatarsal artery and dorsal pedal artery.But it is difficult to separatethe first plantar metatarsal artery,thus,the surgical anatomy of this arterywas studied in 68 feet of adult cadavers (among them 13 were vascular cast spe-cimens).The course of the artery is constant.Its caliber is about 1.5 mm.Thisartery may be divided into five types in form,and the proximal part of the arteryis deeper than the distal.In order to show the first plantar metatarsal artery,thelateral head of the flexor hallucis brevis and the attachment of the adductor hallucishave to be severed.The medium part of the artery is not situated at the first me-tatarsal space,but at the plantar aspect of the first metatarsal (?)
