老年人骨质疏松性髋部骨折术后应用阿伦膦酸钠联合骨化三醇胶丸的疗效

Study on effect of senile osteoporotic hip fracture after operation Allen phosphonic acid sodium plus rocaltrol

目的探讨老年人骨质疏松性髋部骨折术后应用阿伦膦酸钠联合骨化三醇胶丸(罗盖全)的治疗效果。方法选取2011年2月至2012年2月在该院骨科住院治疗的老年骨质疏松髋部骨折患者38例。随机分为治疗组和对照组,每组19例。两组患者均给予钙尔奇D常规治疗,对照组给予阿伦膦酸钠,治疗组同时给予阿伦磷酸钠联合罗盖全,对比两组患者治疗前后骨密度、骨代谢指标以及骨形态计量学和药物治疗相关不良反应。结果治疗12个月后,治疗组4个部位的骨密度均明显高于治疗前,对照组腰椎1~4和Wards三角骨密度明显高于治疗前(P<0.05);治疗12个月后治疗组4个部位的骨密度值均明显高于对照组(P<0.05)。治疗组治疗12个月后Pyd/Cr比值明显低于治疗前(P<0.05)。骨形态学显示,治疗组患者治疗12个月后,骨小梁加粗、间隙变窄,皮质骨变厚、密度增加;对照组患者骨小梁变细、间隙加宽,皮质骨变薄、密度减少;计量学显示,治疗12个月后治疗组各指标均增加,对照组均减少。两组治疗期间均无严重药物相关不良反应发生。结论阿伦膦酸钠联合罗盖全能有效降低老年骨质疏松性髋部骨折患者术后的骨转换,抑制骨吸收,增加骨密度,且无严重不良反应。

Objective To explore the treatment effect of Allen phosphonic acid sodium plus Rocaltrol on senile osteoporotic hip frac -ture after operation .Methods 38 patients with hip fracture were randomly divided into treatment and control groups , each group had 19 ca-ses.Two groups of patients were given Caltrate D routine therapy , control group at the same time to give Allen a phosphonic acid sodium , the treatment group also received Allen sodium phosphate combined with Rocaltrol .Results After 12 months of treatment , the bone density of 4 parts of the treatment group was obviously higher than that before treatment , the bone densities of lumbar 1~4 and Wards triangular of con-trol group were significantly higher than those before treatment (P<0.05);bone density after 12 months of treatment the treatment group of 4 parts of the values were significantly higher than those in control group (P<0.05).The treatment group after treatment for 12 months, Pyd/Cr was lower than that before treatment, the difference had statistical significance (P<0.05).Bone morphology showed, the treatment group patients after 12 months, bone little Liang Jiacu, space narrowing, cortical bone thickness, density were increased.Conclusions Al-len phosphonic acid sodium combined with rogie Al might reduce conversion postoperative bone of osteoporotic hip fracture , inhibit bone re-sorption, increase bone density , and has no serious adverse reactions .
