可溶性ST2在急性冠脉综合征患者预后评估中的价值 被引量:6

摘要 目的评估可溶性ST2(s ST2)水平对急性冠脉综合征(ACS)患者预后判定的应用价值。方法符合美国心脏病学会(ACC)ACS患者102例,失访2例。测定外周血s ST2、pro-BNP、超敏C反应蛋白(hs-CRP)、超敏肌钙蛋白(TNT)。研究随访6个月,通过门诊或电话随访患者病情变化及心脏事件。结果 1各组间s ST2、pro-BNP水平存在显著性差异(P<0.05,P<0.01)。2s ST2与血糖,总胆固醇(TC)及CK-MB峰值呈正相关,与高密度脂蛋白(HDL)呈负相关。3随访期,心脏事件发生率与s ST2水平正相关。4s ST2检测的敏感性及特异性略优于超敏TNT、hs-CRP及pro-BNP。预测心力衰竭事件、ACS、心血管意外死亡最敏感的生化指标分别是s ST2,超敏TNT,s ST2。结论 ACS患者s ST2水平是预测ACS患者预后的独立指标,不受年龄、肝肾功能、心肌酶谱水平、血糖、血脂、左室射血分数(LVEF)、血白细胞等的影响。综合监测s ST2、超敏TNT,hs-CRP和proBNP,可提高评估ACS预后的敏感性及特异性。
