介入治疗过程中联合应用血栓抽吸导管与替罗非班治疗急性心肌梗死高血栓负荷患者的疗效及安全性 被引量:12

摘要 目的观察急性心肌梗死(AMI)高血栓负荷患者行急诊冠状动脉介入治疗(PCI)过程中联合应用血栓抽吸导管与替罗非班的临床疗效与安全性。方法连续选择2009~2013年符合急诊PCI治疗条件的高血栓负荷的患者283例,其中血栓抽吸导管(TAC)+替罗非班+常规手术组(A组)76例,替罗非班+常规手术组(B组)72例,常规手术组(C组,对照组)135例。比较三组患者手术前后的罪犯血管TIMI血流变化、术后1 w及1年左室射血分数(LVEF)及左室舒张末期内径(LVEDD)、住院期间及术后1年主要心脏不良事件(MACE)的发生率、出血性并发症发生率。结果(1)PCI术后即刻达TIMIⅢ级血流比例三组间有差异(P<0.05);(2)术后1 w及1年LVEF及LVEDD,三组间均有显著性差异(P<0.05);(3)患者住院期间总MACE发生率三组间差异不明显(P>0.05);但PCI术后1年总MACE发生率三组间有显著差异(P<0.05),其中A组住院期间及术后1年MACE发生率均最低(P<0.05);(4)三组间大出血发生率无明显差异(P>0.05)。结论 TAC联合替罗非班治疗可显著改善急性心肌梗死高血栓负荷患者术后TIMI血流、心功能,降低MACE发生率,不增加患者大出血风险。
