早发冠心病患者血清骨保护素水平及其临床危险因素 被引量:20

摘要 目的研究早发冠心病患者血清骨保护素水平及相关危险因素。方法以胸痛就诊患者455例,根据冠脉造影结果确诊为冠心病者370例,冠心病组按年龄(男≤55岁,女≤65岁)分为早发冠心病组267例,晚发冠心病组103例。早发冠心病组按年龄分为3个亚组,1亚组(男≤35岁,女≤45岁)56例,2亚组(35<男≤45岁,45<女≤55岁)84例,3亚组(45<男性≤55岁,55<女性≤65岁)127例;冠脉造影正常者85例为对照组。详细记录患者的一般情况包括:年龄、性别、体重指数、吸烟史、饮酒史、高血压病史、糖尿病病史、血脂异常病史、冠心病家族史等危险因素。采用酶联免疫方法检测血清骨保护素水平,经统计分析血清骨保护素和早发冠心病及相关危险因素相关性。结果 1对照组与冠心病组在年龄、性别等各个危险因素比较存在显著性差异(P<0.05),骨保护素水平与冠心病发病呈正相关,与早发冠心病亦呈正相关。2早发冠心病组在性别、冠心病家族史、高血压病史、糖尿病病史、吸烟等方面与晚发冠心病组比较均有统计学差异(P<0.05);早发冠心病组在高敏C-反应蛋白、体重指数、饮酒、骨保护素等方面与晚发冠心病组无统计学差异(P>0.05)。晚发冠心病组在危险因素个数、血脂异常、尿酸等方面与早发冠心病组存在显著差异(P<0.05)。3早发冠心病组中3亚组各个危险因素较晚发冠心病组各危险因素无统计学差异(P>0.05);1亚组在性别、吸烟、冠心病家族史与晚发冠心病组存在显著差异(P<0.01)。结论血清骨保护素水平可预示冠脉病变进展;性别、吸烟、冠心病家族史为早发冠心病最危险因素;早发冠心病年龄界限应该提前。
