三维计算机断层血管造影在诊断老年术后残余或复发性动脉瘤中的价值 被引量:1

摘要 目的比较三维计算机断层血管造影(3D-CTA)与当前的"金标准"数字减影造影术(DSA)在术后残余或复发性动脉瘤诊断中各自准确性。方法对56例行开颅动脉瘤夹闭术的老年患者进行3D-CTA检查,用ROC曲线来比较3D-CTA与DSA的诊断性能。结果 56例老年患者中有69个动脉瘤被发现,DSA检测到8个残余脑动脉瘤,3D-CTA检测到7个残余脑动脉瘤。3D-CTA的敏感性和特异性为81.25%和97.92%,准确度为95.54%。ROC曲线下面积(AUC)是0.928,ROC分析发现3D-CTA诊断性能好。结论研究认为3D-CTA可作为老年病人术后残余动脉瘤检测的替代方式。3D-CTA的敏感性和特异性能够十分接近传统DSA检测方法。3D-CTA作为一种性价比高、非侵入性的方法,非常适用于老年病人,对初始和长期评估残余动脉瘤可以替代DSA的检出手段。
