高中学生胃食管反流病和躯体化症状的相关性研究

Relationship between gastroesophageal reflux disease and somatic symptoms in senior middle-school students

摘要 目的探讨高中学生胃食管反流病(GERD)发病及其与躯体化症状的关系。方法随机选取379名在校高中年级学生,通过调查问卷的方式,进行GERD自评量表(Gerd Q)和躯体化症状自评量表评分。结果所有调查对象总体Gerd Q评分为(5.71±1.95)分;其中男性为(5.71±1.67)分,女性为(5.70±2.27)分。男性和女性学生的Gerd Q评分无统计学差异(t=0.492,P>0.05)。符合GERD诊断标准占8.18%,其中男性5.61%,女性11.52%,男、女GERD阳性率差异具有统计学意义(χ2=45.132,P<0.01)。总体躯体化症状评分为(29.84±7.26)分;其中男性为(30.11±8.15)分,女性为(29.52±6.10)分,男性和女性学生的躯体化症状评分无统计学差异(t=0.671,P>0.05)。符合GERD诊断的患者其躯体化症状评分为(38.43±10.23)分,显著高于非GERD患者的躯体化症状评分[(28.22±6.45)分,t=3.013,P<0.01]。Gerd Q和躯体化症状的评分存在正相关(r=0.357,P<0.01),其中男性学生二者的相关性明显(r=0.498,P<0.01),而女性无相关(r=0.224,P=0.183)。结论高中学生可发生GERD,与躯体化症状相关,提示其治疗应该首先选择通过心理辅导减轻心理压力和躯体化症状而非药物治疗。 Objective To investigate the relationship between gastroesophageal reflux disease and somatic symptoms in senior middle-school students. Methods Gerd Q and somatic self-rating scale were used in 379 randomly selected senior middle-school students. Results The Gerd Q score in total students was 5.71±1.95; the score in male students was 5.71±1.67 while 5.70±2.27 in female students(t=0.492, P>0.05). 5.61% of male while 11.52% of female students(χ2=45.132, P<0.01) came up to GERD diagnosis. The somatic self-rating score in total students was 29.84±7.26; the sores of male and female students had no statistical significance(t=0.671, P>0.05); but the score of GERD students(38.43±10.23) was significant higher than that of no-GERD students(28.22±6.45)(t=3.013, P<0.01). There was a positive relationship between Gerd Q score and somatic self-rating score(r=0.357, P<0.01) in total students, especially in male students(r=0.498, P<0.01) rather than female students(r=0.224, P=0.183). Conclusion Senior middle-school students could occur gastroesophageal reflux symptoms, which co-relate to somatic symptoms.
